目的 评价18F-FDG PET/CT对肾透明细胞癌Fuhrman分级的预测价值。方法 回顾性分析43例病理证实的肾透明细胞癌患者的18F-FDG PET/CT图像,分别应用目测法、最大标准摄取值(SUVmax)、肝脏和降主动脉的标准摄取比值(SURliver、SURaorta)对病灶的18F-FDG摄取进行评价。与病理结果对照,判定其对高级别(Fuhrman Ⅲ~Ⅳ级)病变的预测价值并进行各参数间的两两比较。结果 43例中的41例非囊性病变纳入后续分析。低、高级别组之间的SUVmax、SURliver及SURaorta差异均有统计学意义(t=-4.828、-5.630、-5.136,P均<0.001)。目测法、SUVmax、SURliver、SURaorta的曲线下面积分别为0.75、0.87、0.87、0.91,灵敏度分别为62.50%、87.50%、87.50%、87.50%,特异度分别为87.88%、84.85%、87.88%、87.88%。SURaorta与目测法的ROI曲线差异有统计学意义(P=0.018 8)。结论 18F-FDG PET/CT对肾透明细胞癌的Furhman分级具有一定的预测价值,SURaorta是较好的指标。 |

Objective To evaluate the value of 18F-FDG PET/CT in the prediction of Fuhrman grade in renal clear cell carcinomas. Methods Totally 43 cases of pathologically confirmed renal clear cell carcinoma were retrospectively analyzed. The efficiencies of visual assessment, maximum standard uptake value (SUVmax), liver and aorta standarized uptake ratio (SURliver and SURaorta) in differentiating high grade (Fuhrman Ⅲ-Ⅳ) from low grade (Fuhrman Ⅰ-Ⅱ) were calculated and compared. Results Forty-one noncystic lesions were included in the following analysis. SUVmax, SURliver and SURaorta between low and high Fuhrman grade were statistically different (t=-4.828, -5.630, -5.136, all P<0.001). The AUCs of visual assessment, SUVmax, SURliver and SURaorta were 0.75, 0.87, 0.87 and 0.91, while their sensitivity and specificity were 62.50%, 87.50%, 87.50%, 87.50% and 87.88%, 84.85%, 87.88%, 87.88% respectively. The ROCs of SURaorta and visual assessment were statistically different (P=0.0188). Conclusion 18F-FDG PET/CT is useful in detecting Fuhrman grade of renal cell carcinomas, and SURaorta can be a promising parameter. |
