| 目的 通过与传统二维高频超声及术中超声的对比,探讨超声自动全容积扫查系统(AVSS)预测瘢痕子宫前壁下段肌层厚度的价值。方法 对24名预行剖宫产的晚孕期瘢痕子宫孕妇,于手术前一天分别进行AVSS成像及二维高频超声(2DHF)扫查,记录图像并分别测量子宫前壁下段肌层厚度;次日,将术中高频超声直接放置于子宫瘢痕区域测量肌壁厚度,并与术前AVSS及2DHF结果对比分析。结果 AVSS系统可以显示孕妇腹壁下段的皮肤、皮下脂肪及腹直肌结构;同时可以清晰显示子宫浆膜层、肌层与黏膜层的结构关系。AVSS的测值与术中超声测值呈正相关(r=0.76, P<0.01),与术中超声的测量差值集中在≤0.5 mm范围内。结论 AVSS测量晚孕期瘢痕子宫前壁下段肌层厚度重复性好,其测值更接近术中实际厚度。 |

| Objective To explore the value of automated volume scanner system (AVSS) in predicating the muscle thickness of lower uterine segment by comparing with tradition 2-dimensional high frequency (2DHF) and intraoperative ultrasound. Methods The images were recorded and the muscle thickness of the lower segment of uterine anterior wall was measured in 24 pregnant women with previous cesarean section. At the second day, intraoperative ultrasound was directly placed on the scar region of uterus to measure the muscle thickness, and the result was compared with AVSS and 2DHF. Results AVSS system showed the skin, subcutaneous fat and rectus abdominis structure of the abdominal wall lower segment of pregnant woman; at the same time, it clearly indicated the structure and relevance of uterus serosa, muscular layer and mucous layer. Measuring value of AVSS had positive correlation with intraoperative ultrasonic measurement result (r=0.76, P<0.01), and the difference between measuring value of AVSS and intraoperative measuring value was ≤ 0.5 mm. Conclusion AVSS in measurement of lower uterine segment thickness of pregnant women with previous cesarean section has good repeatability, and the measuring value is more close to the intraoperative measuring value. |
