目的 探讨超微血管成像(SMI)技术鉴别诊断颈部淋巴结病变的临床应用价值。方法 回顾分析71例颈部淋巴结病变患者的超声检查资料,与病理结果相对照,比较二维灰阶超声联合CDFI、二维灰阶超声联合SMI鉴别诊断颈部淋巴结病变的准确率,同时采用SMI技术分析颈部病变淋巴结的微血管分布类型。结果 71例患者共71枚颈部淋巴结,其中恶性淋巴结39例,包括淋巴瘤19例、转移性淋巴结20例,良性淋巴结32例;二维灰阶超声联合SMI的诊断准确率[88.73%(63/71)]高于二维灰阶超声联合CDFI[76.06%(54/71);P<0.05];良性淋巴结微血管分布以Ⅰ型(淋巴门血供型)和Ⅲ型(周边血供型)为主,恶性淋巴结内微血管分布以Ⅱ型(内部点条血供型)和Ⅳ型(混合血供型)为主。结论 SMI技术较CDFI技术有更高的血管显示率,通过SMI技术对颈部淋巴结内微血管分布类型的分析,有助于提高对颈部淋巴结病变定性诊断的准确率。 |

Objective To discuss clinical application value of the differential diagnosis in cervical lymph node lesions by using of superb microvascular imaging (SMI). Methods The ultrasound data of 71 cases of cervical lymph node enlargement was retrospectively analyzed. According to pathological findings, the accuracy in differential diagnosis of benign or malignant lymph modes were compared by two-dimensional gray-scale ultrasonography combined with CDFI and two-dimensional gray-scale ultrasonography combined with SMI. Microvascular distribution types of lymph node using SMI technology was analyzed. Results Among 71 cervical lymph nodes, there were 32 benign lymph nodes and 39 malignant lymph nodes including 19 lymphoma and 20 metastasis of lymph node. The differential diagnostic accuracy of two-dimensional gray-scale ultrasonography combined with SMI (88.73%[63/71]) was higher than that combined with CDFI (76.06%[54/71]; P<0.05). According to SMI technology, the main microvascular distribution types in benign lymph nodes were type Ⅰ (hilar vascular) and type Ⅲ (surrounding vascular), while that in malignant lymph nodes were type Ⅱ (interior point and strip vascular) and type Ⅳ (mixed vascular). Conclusion The vascular display rate of cervical lymph nodes of SMI is higher than that of CDFI. The analysis of microvascular distribution types by SMI is helpful in promoting the accuracy of differential diagnosis of cervical lymph nodes lesions. |
