      目的 探讨采用256层螺旋CT行"双低"(低管电压、低浓度对比剂)及"个性化双低"头颈部CTA的可行性.方法 将90例患者随机分为常规"双低"组(A组:管电压100 kV,对比剂 300 mgI/ml、60 ml)、"个性化双低"组(B组:管电压100 kV,对比剂 300 mgI/ml、个体化对比剂用量)及对照组(C组:管电压120 kV,对比剂 370 mgI/ml、60 ml),测量、计算及分析各组动脉CT值、辐射剂量、图像质量,比较两组双低组对比剂用量.结果 ①3组动脉CT值差异均有统计学意义,且A、B组均高于C组(P均<0.05).②除右侧双锁骨下动脉(SA)外,其余各动脉图像质量评分差异均无统计学意义(P>0.05).③A、B组辐射剂量明显低于C组(P<0.05).④与常规"双低"组相比,个性化"双低"组45~60 kg及61~75 kg体质量节段患者对比剂用量下降,>75 kg体质量患者用量上升(P均<0.05),但两组总体对比剂平均用量差异无统计学意义(P>0.05).结论 采用256层螺旋CT行"双低"及"个性化双低"头颈部CTA既能满足图像诊断要求,又能同时降低辐射剂量和对比剂用量.
英文摘要:
      Objective To evaluate the feasibility of low tube voltage, low concentration and individual contrast medium volume in cerebral and carotid CTA with 256-slice spiral CT scanner. Methods Totally 90 patients who underwent cerebral and carotid CTA were randomly divided into 3 groups by the voltage, concentration and volume of contrast medium (group A: 100 kV, 300 mgI/ml, 60 ml; group B: 100 kV, 300 mgI/ml, individual volume; group C: 120 kV, 370 mgI/ml, 60 ml). Image quality, the CT value and radiation dose were compared among 3 groups. The volume of contrast medium was compared between group A and B. Results ①There were statistical differences among 3 groups in CT value of all arteries, which were higher in group A and B than that in group C (all P<0.05). ②There was no significant difference in the image quality of all arteries except for the right subclavian artery among 3 groups. ③The radiation dose in group C was the most lowest in three groups (P<0.05). ④Comparing to group A, the volume of contrast medium was declined in those patients weighing 45-60 kg and 61-75 kg in group B. It was ascended in those patients weighing more than 75 kg (both P<0.05). There was no statistical difference in the volume of contrast medium between group A and B (P>0.05). Conclusion It can reduce the radiation dose and the volume of contrast medium, using low tube voltage, low concentration and individual contrast medium volume in cerebral and carotid CTA with 256-slice CT.
