| 目的 探讨自体脂肪移植隆胸术后脂肪坏死的乳腺X线摄影及MRI表现.方法 收集自体脂肪移植隆胸术后发生脂肪坏死的患者资料33例,所有患者均于术后3~15年分别接受乳腺X线摄影、MR检查,分析脂肪坏死的影像学特征.结果 33例患者中21例X线摄影表现为类圆形脂肪密度病灶,其中12例为多发病灶,邻近乳腺腺体受压移位,14例有"囊壁"伴点状、弧形钙化;12例表现为边界清晰的等密度结节.23例MR表现为单发或多发薄壁富含脂肪病灶,其中19例囊壁厚度均匀,4例囊壁厚度不均;10例表现为边界清晰的等信号结节,动态增强扫描呈渐进性环形强化,其中2例DWI呈高信号.结论 自体脂肪移植隆胸术后脂肪坏死以"囊变"合并钙化为乳腺X线摄影及MRI的特征性表现,有助于与恶性肿瘤相鉴别. |

| Objective To evaluate the mammography and MRI findings of fat necrosis after autologous fat grafting breast augmentation. Methods Thirty-three patients underwent fat necrosis after breast augmentation by autologous fat transplant were enrolled. All patients underwent mammography and MRI after operation of 3-15 years. Imaging findings of postoperative breast were observed. Results In mammography, 21 cases characterized by oval lesions with fat density; 12 cases of them with multiple lesions extruding adjecent breast tissue; 14 cases of them characterized by cystic wall with calcified. Another 12 cases characterized by isodensity nodules. In MRI, 23 cases characterized by annular enhancement lipid lesions, of which 19 showed uniform wall thickness, 4 cases displayed wall thickness nonuniformity; 10 cases characterized isointensity lesions with the clear boundary, of which 2 cases displayed hyperintensity on DWI. Conclusion Calcified with cyst is characteristic features of fat necrosis after autologous fat grafting breast augmentation in X-ray mammography and MRI, which is helpful to differential diagnosis of malignant tumors. |
