      目的 探讨CT诊断涂阴肺结核及其在疗效评估中的价值.方法 收集涂阴肺结核患者167例(涂阴组)、涂阳肺结核患者63例(涂阳组).涂阴组(治疗前、治疗2个月、治疗结束)和涂阳组均接受CT检查.分析记录两组病灶分布及CT征象,并进行统计学分析.结果 涂阴组磨玻璃影、肺段以上实变、单发空洞、多发空洞、支气管壁增厚的检出率均明显低于涂阳组(P均<0.05).涂阴组病灶双肺分布的比率明显低于涂阳组[19.76%(33/167)、61.90%(39/63),χ2=37.78,P<0.05].涂阴组检出4种以上活动性CT征象的患者比率明显低于涂阳组[39.52%(66/167)、74.60%(47/63),χ2=22.53,P<0.05].涂阴组与治疗前比较,治疗后活动性肺结核CT征象(除空洞外)检出率均明显降低(P均<0.05),非活动性肺结核CT征象检出率均明显增高(P均<0.05).结论 CT在涂阴肺结核的诊断及疗效评估中具有很高的应用价值.

      Objective To explore the value of CT in diagnosis of smear-negative pulmonary tuberculosis and assessment of curative effect. Methods Totally 167 patients with smear-negative pulmonary tuberculosis (smear-negative group) and 63 patients with smear-positive pulmonary tuberculosis (smear-positive group) were involved. Smear-negative group (before treatment, two months after treatment and the end of treatment) and smear-positive group both underwent CT examination. Lesion distribution and CT features were analyzed, and statistical analysis was performed. Results The detection rate of ground-glass opacity, consolidation of area larger than pulmonary segment, single cavity, multiple cavity, thickening of bronchial wall in smear-negative group were all significantly lower than those of smear-positive group (all P<0.05). The rate of double lung lesion distribution in smear-negative group was significantly lower than that of smear-positive group (19.76%[33/167], 61.90%[39/63]; χ2=37.78, P<0.05). The detection rate of patients with four and above kinds of CT features in smear-negative group was significantly lower than that of smear-positive group (39.52%[66/167], 74.60%[47/63], χ2=22.53, P<0.05). In smear-negative group, active CT features (except for cavity) all decreased significantly at the end of treatment than before treatment (all P<0.05), while all inactive CT features increased significantly (all P<0.05). Conclusion CT has a high application value in diagnosis of smear-negative pulmonary tuberculosis and assessment of curative effect.
