      目的 探讨640层容积CT 4D-CTA技术在缺血性脑血管病成像中的临床应用价值.方法 对45例拟诊为缺血性脑血管病的患者进行640层CT扫描,经Toshiba Aqulilion One 320工作站获得4D-CTA、3D-MIP、VR、4D-CTP图像;观察分析图像质量、血管狭窄程度及CTP参数特点.结果 45例中41例均获得高质量的4D-CTA图像,优良率为91.11%(41/45).36例患者脑血管狭窄或闭塞、2例为大脑中动脉支架术后、1例为动脉瘤术后、1例为烟雾病患者,5例患者正常.4D-CTA显示8例患者的侧支循环较3D-MIP多.10例CTP未见异常,35例异常灌注主要分3类:第Ⅰ类(n=11)为延迟时间(DLY)、达峰时间(TTP)、通过时间(MTT)均升高,脑血流量(CBF)、脑血容量(CBV)正常或轻度升高;第Ⅱ类(n=9)为DLY、TTP、MTT升高,CBF、CBV轻度降低;第Ⅲ类(n=15)为DLY、TTP、MTT升高,CBF、CBV明显降低.结论 640层容积4D-CTA可动态观察颅内血管形态、评价脑组织血流灌注情况,可为缺血性脑血管病变诊断与治疗提供有价值的信息.

      Objective To investigate the application value of 4D-CTA in cerebral vascular diseases with 640-slice volume CT. Methods Forty-five patients diagnosed as ischemic cerebrovascular diseases were examined with 640 slice-CT scanner. 4D-CTA, 3D-MIP, VR, 4D-CTP images were generated on post-processing workstation of Toshiba Aqulilion One 320. Images quality, vascular stenosis degree and CTP parameters were analyzed. Results Forty-one cases got better quality 4D-CTA image of 45 patients, the ratio of superior images was 91.11% (41/45). Thirty-six cases had vessel stenosis or occlusion, 2 cases were with stents of cerebral middle artery, 1 case was after surgery of aneurysm, 1 case was with Moyamoya, 5 cases were normal. 4D-CTA showed more visible collateral vessels than 3D-MIP in 8 cases. Ten cases were normal in CTP, but three patterns of abnormity perfusion were identified: ①Delay time (DLY), time to peak (TTP) and mean transit time (MTT) increased, cerebral blood flow (CBF) and cerebral blood volume (CBV) were normal or increased slightly (n=11); ②DLY, TTP and MTT increased, CBF and CBV slightly decreased (n=9); ③DLY, TTP and MTT increased, CBF and CBV decreased obviously (n=15). Conclusion 4D-CTA of 640 slice volume CT can display structure of vessels dynamically and the function status of the whole brain, and provide valuable information for the diagnosis and treatment of cerebral vascular diseases.
