      目的 探讨超声弹性成像量化分析评价肌肉损伤后肌肉组织硬度的价值。方法 选取20例腓肠肌损伤患者(损伤组)及80名健康志愿者(正常组)。将正常组按年龄段划分为青年亚组(<30岁),中年亚组(30~60岁),老年亚组(>60岁),青年亚组25例,中年亚组30例,老年亚组25例。均行常规二维超声及超声弹性成像,并进行弥散性定量分析。比较正常组男女性间、各年龄段亚组受检者间、以及损伤组患侧与健侧间小腿肌肉弹性参数的差异。对损伤组患侧与健侧间存在统计学差异的参数与二维超声测得的液性区容积行Pearson相关性分析。结果 损伤组患侧与健侧间,11个参数中,除应变均值、蓝色区域所占百分比(AREA%)、复杂度的差异有统计学意义外(P均<0.001),标准差、偏度、峰度、对比度、均等性、杂乱度、一致性、相关性的差异均无统计学意义(P均>0.05)。正常组男、女性及3个亚组间11个弹性参数的差异均无统计学意义(P均>0.05)。损伤组中患侧应变均值与液性区容积呈负相关(r=-0.81,P<0.01)。结论 超声弹性成像弥散性定量分析可在二维超声的基础上提供更为客观、准确的肌肉硬度的定量数据及图像信息。

      Objective To explore the diagnostic value of quantitative analysis of ultrasound elastography for assessing the changes of the muscle hardness after the muscle injuring. Methods Totally 20 patients with calf injuries(injury group) and 80 healthy volunteers(normal group) were selected. The normal group was subdivided into the following 3 age groups:Young subgroup(<30 years old), middle-aged subgroup(30-60 years old) and elderly subgroup(>60 years old), young subgroup and elderly subgroup have 25 subjects, respectively, while the middle-aged subgroup have 30 subjects. All subjects were examined by two-dimensional ultrasound and elastosonography, then subjected to diffuse quantitative analysis. The parameters of differences of calf muscle elasticity were evaluated by comparing male vs female, 3 age subgroups in normal group, and healthy limb vs injured limb in the injury group. Pearson's correlation analysis was performed between the injury group with statistically significant different parameters and volume of liquid area measured by two-dimensional ultrasound. Results Of all the 11 parameters of healthy limb and injured limb in the injury group, only the 3 parameters, i.e. strain mean value, the percentage of blue area(AREA%) and order of complexity had statistically significant difference(all P<0.001), the other parameters, such as standard deviation, kurtosis, skewness, contrast, equivalence, mixed and disorderly, consistent degrees, correlation, consistency and correlation had no statistically significant difference(all P>0.05). There was also no statistically significant difference in 11 elasticity parameters in the normal group(all P>0.05). Negative correlation was found between the strain mean value of the injured limb in injury group and volume of liquid area(r=-0.81, P<0.01). Conclusion On the base of two-dimensional ultrasound, diffuse quantitative analysis of elastography can be used to provide more objective and accurate quantitative data and image information on muscle tissue hardness.
