      目的 应用时间-空间关联成像(STIC)-M模式测量三尖瓣瓣环收缩期位移(TAPSE),探讨TAPSE定量评价胎儿右心收缩功能的价值。方法 应用M型超声心动图及STIC-M模式成功测量正常中晚孕胎儿TAPSE 170胎,每个孕周10胎,比较两种方法测量结果是否存在差异。分析TAPSE与孕周(GA)、胎儿体质量预测值(EFW)、三尖瓣口舒张早期血流峰值速度(E)、舒张晚期血流峰值速度(A)的相关性。结果 应用STIC-M模式和M型超声心动图测量TAPSE结果差异无统计学意义。应用STIC-M模式测得的TAPSE与GA、EFW、E、A呈显著相关性(P均<0.001)。结论 正常中晚孕胎儿TAPSE随GA、EFW增加而增长,与E、A等传统右心功能评价参数显著相关。STIC-M模式测量TAPSE具有简单、可重复性强、受胎儿体位影响小的特点,可作为一种新的准确评估胎儿右心功能的方法。
英文摘要:
      Objective To quantitatively study of right ventricular systolic function in the normal second and third trimester fetuses by tricuspid annular plane systolic excursion (TAPSE) measured by spatio-temporal image correlation-M (STIC-M) mode.Methods TAPSE was successfully measured by using conventional M-mode echocardiography and STIC-M mode on 170 normal second and third trimester fetuses and each gestation age (GA) had 10 fetuses.The two methods of measurement were compared.Meanwhile the correlations between TAPSE and other parameters such as GA,estimated fetal weight (EFW),tricuspid early diastolic peak E-wave velocity (E),tricuspid late diastolic peak A-wave velocity (A) were analyzed.Results No significant difference was found in measures obtained with conventional M mode and STIC-M mode.TAPSE measured by STIC-M mode was correlated with GA,EFW,E and A significantly (all P<0.001).Conclusion In normal second and third trimester fetuses,TAPSE increases with the growth of GA,EFW,and it has strong correlation with E and A.TAPSE measured by STIC-M mode is almost not affected by fetal position,and it may become a new promising modality to evaluate right ventricular function simply and accurately.
