      目的 探讨扩散加权成像(DWI)及动态增强(DCE)MRI鉴别诊断子宫肉瘤与变性子宫肌瘤的价值。方法 回顾性分析本院经病理证实的28例子宫肉瘤和30例变性子宫肌瘤患者临床及影像资料,所有患者均接受DWI和DCE MRI。分析病变的表观扩散系数(ADC)、达峰时间(TTP)、最大对比增强率(MCER)和时间-信号强度曲线(TIC)类型,比较不同肿瘤间的差异。评价DWI和DCE鉴别诊断子宫肉瘤与变性子宫肌瘤的效能。结果 子宫肉瘤与变性子宫肌瘤的ADC、TTP、MCER[(0.86±0.09)×10-3mm2/s vs (1.44±0.16)×10-3mm2/s,(44.18±5.04)s vs (55.20±4.66)s,(240.98±24.62)% vs (145.13±16.59)%]差异均有统计学意义(P均<0.01)。绘制ROC曲线后鉴别子宫肉瘤与变性子宫肌瘤的ADC、TTP和MCER阈值分别为1.01×10-3mm2/s、49 s和208.3%。子宫肉瘤中22例TIC呈Ⅰ型,4例TIC呈Ⅱ型,2例TIC呈Ⅲ型;变性子宫肌瘤中26例TIC呈Ⅰ型,4例TIC呈Ⅱ型。结论 子宫肉瘤的DWI及DCE MRI表现具有一定特征性,结合相关参数有助于子宫肉瘤与变性子宫肌瘤的鉴别诊断。

      Objective To assess the value of DWI and dynamic contrast-enhanced (DCE) MRI in differential diagnosis of uterine sarcomas and degenerated leiomyomas.Methods A total of 28 uterine sarcomas and 30 uterine degenerated leiomyomas confirmed by pathology were analyzed retrospectively.All of patients underwent DWI and DCE-MRI.The apparent diffusion coefficient (ADC) value,time to peak (TTP),maximum contrast enhancement ratio (MCER) and time signal-intensity curves (TIC) type were measured and compared between uterine sarcomas and degenerated leiomyomas.The diagnostic efficacy of DWI,DCE MRI in distinguishing uterine sarcomas and degenerated leiomyomas were calculated.Results The ADC value ([0.86±0.09]×10-3mm2/s),TTP([44.18±5.04]s) and MCER([240.98±24.62]%) of 28 uterine sarcomas were statistically different from 30 degenerated leiomyomas([1.44±0.16]×10-3mm2/s,[55.20±4.66]s,[145.13±16.592]%).The cut-off value of ADC,TTP,and MCER were respectively 1.01×10-3mm2/s,49 s and 208.3% in differential diagnosis of uterine sarcomas and degenerated leiomyomas.There were 22Ⅰtype TICs,4Ⅱ type TICs and 2Ⅲ type TICs in 28 uterine sarcomas.There were 26Ⅰtype TICs and 4Ⅱ type TICs in 30 degenerated leiomyomas.Conclusion DWI and DCE MRI features of uterine sarcomas are characteristic,and the combination of ADC value,TTP and MCER is helpful for the differential diagnosis of uterine sarcomas and degenerated leiomyomas.
