目的 探讨单源双能量CT的优化CNR技术及物质分离技术对中心型肺癌肿块与远端肺不张组织的鉴别能力。方法 采用单源双能量CT对27例中心型肺癌伴肺不张的初治患者行平扫、增强后30 s及90 s单能量扫描。获取优化单能量图像及碘基图,分析肿瘤及远端肺不张区域的碘浓度差异。结果 增强后30 s时,最佳单能量图像、最佳单能量图像联合碘基图可较好地显示瘤、肺界面分别为9例(9/27,33.33%)、16例(16/27,59.26%),差异无统计学意义(χ2=3.65,P=0.06)。增强后90 s时,最佳单能量图像、最佳单能量图像联合碘基图可较好地显示瘤、肺界面分别为16例(16/27,59.26%)、26例(26/27,96.30%),差异有统计学意义(χ2=10.71,P=0.001)。增强30、90 s时肿瘤碘含量均低于肺不张组织(t=-6.94、-6.12,P均<0.001)。结论 单源双能量CT的优化CNR技术及物质分离技术对中心型肺癌与肺不张有较好的鉴别能力,有助于临床放疗野的精确划定。 |

Objective To evaluate the performance of optimized CNR single source dual energy CT for differentiating the central lung tumor from the collapsed lung.Methods Twenty-seven patients of central lung with atelectasis were inspected with single source dual energy CT.The optimized monochromatic images and iodine-based material decomposed images were obtained by selecting the optimized CNR,and iodine concentrations of tumor and atelectasis were compared.Results The tumor-atelectasis interface was defined in 16(16/27,59.26%) and 26(26/27,96.30%) patients by using combination of the optimized monochromatic images with iodine-based material decomposed images at 30 s and 90 s delayed phase,respectively,which were 9(9/27,33.33%) and 16(16/27,59.26%) cases by using the optimized monochromatic images alone (χ2=3.65,10.71,P=0.06,0.001),respectively.The iodine concentration of atelectasis at 30 s and 90 s delayed phase were much higher than those of tumors (t=-6.94,-6.12,all P<0.001).Conclusion The differentiation between atelectasis and tumor in central lung cancer can be improved by combination of the optimized CNR technique and material decomposition technique,which is helpful to define the area of radiotherapy. |
