      目的 应用血流向量成像(VFM)技术评价房间隔缺损(ASD)患者术前及术后短期内右心室腔舒张期血流流场变化。方法 选择20名健康志愿者为对照组,20例经手术治疗的Ⅱ孔型ASD患者为ASD组,于手术前后在VFM成像模式下观察右心室腔血流向量图、流线图及涡流图的流场变化规律,对比分析右心室舒张期基底段、中间段及心尖段流场峰值速度(Vp)、峰值流量(Fp)及舒张期正向流量(DQ+)等血流参数变化。结果 与对照组比较,ASD组术前右心室舒张期向量线及流线密集、方向杂乱,三尖瓣前叶及隔叶下方涡流面积增大,术后右心室腔内向量线及流线密集程度减小,方向趋于一致,涡流数减少且面积减小,但较对照组仍有一定差别。ASD患者术后短期内右心室舒张期各节段血流的Vp、Fp及DQ+较术前明显降低,但仍高于对照组(P均<0.05)。结论 ASD患者术后右心室腔舒张期血流流场动力学较术前明显恢复,但短期内仍未恢复至正常状态。VFM技术可用于ASD术后血流动力学状态的监测与随访。

      Objective To assess the changes of the flow field in diastole of the right ventricle before and a short term after surgery in patients with the atrial septal defect (ASD) by the vector flow mapping (VFM).Methods Totally 20 patients with secundum ASD were enrolled as ASD group,while 20 healthy volunteers were chosen as control group.The pre-and post-operative characteristics of the flow field were compared by vector,streamline and vortex modes of the VFM,separately.The parameters at basal,mid and apical levels of right ventricular in the two groups were also recorded and compared,including diastolic peak velocity (Vp),diastolic peak flow (Fp) and diastolic Q+(DQ+).Results Before ASD closure,the ASD group had intensive but disordered vector and stream lines,the vortex area below the tricuspid leaflets in diastole was also larger than the control group.After operation,the line intensity and direction consistence were recovered and the vortex area was reduced to some degree,but still different from the control group.After operation,the Vp、Fp and DQ+ at each level of right ventricle were lower than those before operation,but still higher than the control group (P<0.05).Conclusion The hemodynamics of the flow field in the right ventricle of the ASD patients improved after operation,but still not completely recovered in the short time.The vector flow mapping could be used for the postoperative monitoring and follow-up of the cardiac hemodynamics.
