目的 探讨18F-FDG PET/CT在宫颈癌治疗后血清鳞状细胞癌抗原(SCCAg)再次升高患者中的应用价值。方法 回顾性分析64例宫颈癌手术和(或)放、化疗后血清SCCAg再次升高,临床怀疑复发和(或)转移者行18F-FDG PET/CT全身扫描,查找复发转移灶。以病理学和(或)随诊结果为金标准,计算PET/CT诊断宫颈癌复发和(或)转移的效能,比较不同SCCAg水平下PET/CT诊断准确率的差异。结果 PET/CT发现异常代谢增高灶者50例,阴性表现者14例。PET/CT诊断宫颈癌复发和(或)转移的灵敏度为96.00%(48/50),特异度为85.71%(12/14),阳性预测值为96.00%(48/50),阴性预测值为85.71%(12/14),准确率为93.75%(60/64)。血清SCCAg为1.7~<5 μg/L者PET/CT诊断准确率为90.91%(30/33),≥5.0~70.0 μg/L者准确率为96.77%(30/31),两者比较差异无统计学意义(χ 2=0.92,P=0.34)。结论 18F-FDG PET/CT对宫颈癌治疗后SCCAg再次升高者复发和(或)转移的检出率高,定位准确。 |

Objective To explore the value of 18F-FDG PET/CT in cervical cancer patients with elevated squamous cell carcinoma antigen (SCCAg) during the follow-up. Methods PET/CT images of 64 cervical cancer patients after the treatment with SCCAg elevated and suspision of tumor recurrence and (or) metastases were retrospectively reviewed. The diagnostic efficacy of PET/CT was evaluated based on pathologic findings and (or) follow-up. The difference of diagnostic accuracy of PET/CT with different level of SCCAg was compared. Results PET/CT finding was positive in 50 patients and negative in 14 patients. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of PET/CT in diagnosis recurrence and (or) metastases of tumor was 96.00% (48/50), 85.71%(12/14), 96.00% (48/50), 85.71% (12/14) and 93.75% (60/64). According to the level of SCCAg, the patients were divided into two groups, SCCAg<5 μg/L (n=33) and SCCAg≥5 μg/L (n=31). The accuracy of PET/CT for each group was 90.91% (30/33) and 96.77% (30/31), which showed no significant difference (χ2=0.92, P=0.34). Conclusion 18F-FDG PET/CT is efficient in accurately detecting and locating the posttreatment recurrence and metastases of cervical cancer with elevated SCCAg. |
