目的 探讨剪切波弹性成像技术在评价乳腺癌新辅助化疗(NAC)疗效中的价值。方法 对49例(NAC)患者在化疗前后均进行剪切波弹性成像。记录并将化疗前肿瘤的二维超声及弹性成像指标与实体瘤治疗疗效评价标准指标(RECIST指标)进行单因素及多因素回归分析。结果 化疗前后剪切波弹性模量的最大值、平均值、最小值、比值差异均有统计学意义(P均<0.05)。单因素分析显示年龄、边缘、肿块化疗前后大小差值、化疗前弹性模量参数(最大值、平均值、最小值)均与实体瘤治疗疗效评价标准评价指标有相关性(P均<0.05)。多因素分析显示年龄、边缘、肿块化疗前后大小差值、化疗前最大值与RECIST评价指标有相关性(P均<0.05)。结论 采用剪切波弹性成像技术可以反映乳腺肿块化疗前后的变化,化疗前肿块弹性成像的最大值可用于预测浸润性导管癌NAC效果。 |

Objective To assess shear wave elastography ultrasound in evaluating the efficacy of the neoadjuvant chemotherapy on breast tumors. Methods Totally 49 patients underwent elasticity imaging before and after neoadjuvant chemotherapy. The two-dimensional ultrasound, the elastic imaging index and response evaluation criteria in solid tumors index (RECIST index) of the tumor before chemotherapy were recorded and analyzed with one-way analysis variance and multivative regression. Results The mean values, the minimum values, and the ratio of the shear wave elastic imaging before and after chemotherapy all had statistical significance (all P<0.05). One-way analysis variance showed age, edge, the difference before and after chemotherapy, the shear wave elastography parameters (average, minimum, maximum, the ratio of ultrasound before chemotherapy) were related to response evaluation criteria in solid tumors evaluation index. Multivative regression showed age, edge, the difference before and after chemotherapy, the maximum shear wave elastography ultrasound were independently correlated with RECIST index. Condusion Application of shear wave elastography technology can reflect breast lesions' changes before and after chemotherapy. The maximum shear wave elastography ultrasound could be used to predict response to neoadjuvant chemotherapy with invasive breast cancer. |
