目的 观察不同孕周正常胎儿主动脉弓形态的变化规律,并探讨大动脉转位(TGA)胎儿主动脉弓形态变化。方法 收集150胎正常胎儿(正常组,21~40周,每4个孕周为一个亚组,每亚组30胎)和29例TGA胎儿(病例组,21~24周12例,25~28周12例,29~36周5例)主动脉弓切面图像,利用曲率测量软件分别测量主动脉弓曲率等指标,分析其与孕周的相关性,并对两组相同孕周的胎儿进行统计学分析。结果 正常胎儿主动脉弓最大曲率位置随孕周增加呈上移趋势(P<0.05),二者呈直线正相关(r=0.86,P<0.001),孕周与最大曲率位置的线性方程为Y=0.86X+15.75。随孕周增加TGA胎儿最大曲率位置也呈上移趋势,二者呈正相关(r=0.88,P<0.001),但与相同孕周的正常胎儿相比,其主动脉弓最大曲率位置下移(P均<0.05)。结论 随着孕周的增长,胎儿主动脉弓形态发生显著变化。采用胎儿主动脉弓切面曲率量化主动脉弓形态,有利于丰富胎儿大动脉转位的诊断信息和提高诊断信心。 |

Objective To observe aortic arch morphological changes of normal fetuses in different gestational weeks, and to explore the fetal aortic arch morphological changes in transposition of great arteries (TGA) fetuses. Methods Images of 150 normal fetuses (normal group, 21—40 gestational weeks, four gestational weeks as a subgroup, each n=30) and 29 TGA (case group, 12 cases of 21—24 gestational weeks, 12 cases of 25—28 gestational weeks, 5 cases of 29—36 gestational weeks) fetuses were collected. The maximum curvature locations of aortic arch were measured. The relationship between maximum curvature locations of aortic arch and gestational weeks was analyzed and the differences between normal fetuses and TGA fetuses in the same gestational weeks were statistically compared. Results As the pregnancy progressed, the maximum curvature locations of aortic arch presented upward trend in normal fetuses (P<0.05). The maximum curvature positions of aortic arch and the gestational weeks was positively correlated in normal fetuses (r=0.86, P< 0.001) and the linear equation was Y=0.86X+15.75. As the pregnancy progressed, the maximum curvature locations of aortic arch also presented upward trend in TGA fetuses (P<0.05). The maximum curvature positions of aortic arch and the gestational weeks was positively correlated in TGA fetuses (r=0.88, P< 0.001). Compared with normal fetuses in the same gestational weeks, the maximum curvature locations of aortic arch in TGA fetuses shifted down (all P<0.05). Conclusion As the pregnancy progressed, the morphological change of aortic arch in normal fetuses is significant. Through curvature measurement, the quantificat evaluation of aortic arch can significantly enrich diagnostic information and improve diagnostic confidence of TGA. |
