| 目的 探讨体素内不相干运动(IVIM)MRI评估子宫肌瘤血流灌注及分子扩散状态的不同观察者间的一致性。方法 采用3.0T MR仪对23例子宫肌瘤患者进行T2WI及多b值DWI。2名观察者利用后处理软件选择子宫肌瘤最大层面勾画ROI,使用双指数模型,生成IVIM MRI相关的参数图(D图、D*图、f图),得到数值分布的直方图,分别计算各参数的位于直方图左侧第25%、50%、75%位置的数值及平均值,比较2名观察者测量数据的一致性。结果 23例子宫肌瘤患者中,2名观察者对12例选择最大层面的结果一致;2名观察者间测得的23例子宫肌瘤的Dmean、D25、D50、D75值平均值差值为0~0.07 mm2/s,fmean、f25、f50、f75值平均值差值为0~0.01,D*mean、D*25、D*50、D*75值平均值差值为0~5.38 mm2/s;2名观察者测量的25个IVIM MRI参数的ICC值> 0.9,测量的33个IVIM MRI参数的ICC值> 0.8。结论 采用IVIM MRI评估子宫肌瘤血流灌注及分子扩散状态的不同观察者间的一致性好,这种方法可行。 |

| Objective To explore the inter-observer agreement of intravoxel incoherent motion (IVIM) MRI on evaluation of perfusion and diffusion status of uterine fibroids. Methods All 23 patients of uterine fibroids underwent T2WI and multiple b values DWI. ROI containing the whole lesion was drawn by free hand of two experienced radiologists to generate D, f and D* maps by using bi-exponential model. Then, IVIM MRI parameters were analyzed by using histogram to compute the 25%, 50%, 75% value (n% of the voxel values that form the histogram found to the left) and the mean value. Results The two observers chose the same biggest slice for ROI drawing in 12 of 23 patients. In 23 patients, the different mean value of D25, D50, D75 and the mean D value respectively between the two observers were among 0—0.07 mm2/s, and the different mean value f25, f50, f75 and the mean f value respectively were among 0—0.01, the different mean value of D*25, D*50, D*75 and the mean D* value respectively were among 0—5.38 mm2/s. The intraclass correlation coefficient value was higher than 0.9 in 25 of 36 IVIM MRI parameters and it was higher than 0.8 in 33 of 36 IVIM MRI parameters. Conclusion There is good inter-observer agreement of IVIM MR imaging on evaluation of perfusion and diffusion status of uterine fibroids, and therefore this model is feasible. |
