      目的 探讨应用18F-FDG PET/CT观察慢性肝炎及肝硬化患者肝脏葡萄糖代谢功能变化的价值。方法 回顾性分析50例慢性肝炎患者(肝炎组)、36例肝硬化患者(肝硬化组)及31例肝功能正常者(对照组)的18F-FDG PET/CT资料。比较肝炎组、肝硬化组及对照组全肝平均标准摄取值(SUVmean)、全肝体积(HV)及全肝糖酵解值(GHG)的差异。结果 3组间全肝SUVmean及GHG差异均有统计学意义(P均< 0.01)。肝硬化组SUVmean及GHG低于肝炎组(P均< 0.001)及对照组(P=0.012、0.014),肝炎组SUVmean及GHG高于对照组(P=0.042、0.046)。3组间HV差异无统计学意义(P=0.58)。结论 18F-FDG PET/CT检查指标全肝SUVmean及GHG可有效反映慢性肝炎及肝硬化患者肝脏葡萄糖代谢功能变化。

      Objective To explore the value of 18F-FDG PET/CT in observing hepatic glucose metabolism function changes of chronic hepatitis and liver cirrhosis. Methods 18F-FDG PET/CT data of 117 subjects including 50 patients with chronic hepatitis (hepatitis group), 36 patients with liver cirrhosis (cirrhosis group) and 31 patients without chronic hepatitis or liver cirrhosis (control group) were retrospectively analyzed. Total hepatic mean standardized uptake value (SUVmean), hepatic volumes (HV) and global hepatic glycolysis (GHG) were compared among different groups. Results There were statistical differences of SUVmean and GHG among three groups (all P< 0.01). SUVmean and GHG in cirrhosis group were lower than those in hepatitis group (both P< 0.001) and control group (P=0.012, 0.014). While SUVmean and GHG in hepatitis group were higher than those in control group (P=0.042, 0.046). No statistical difference HV was found (P=0.58). Conclusion 18F-FDG PET/CT indexes as total SUVmean and GHG can effectively reflect the changes of hepatic glucose metabolism function in patients with chronic hepatitis or liver cirrhosis.
