| 目的 探讨超声在成人脐尿管病变临床诊断中的应用价值。方法 回顾性分析经临床及手术病理证实的87例成人脐尿管病变的超声表现。结果 本组成人脐尿管病变的超声表现可分为4类:①脐茸炎或脐窦,病变局限于脐孔中心及周围,呈低至无回声;超声诊断准确率为100%(38/38)。②脐尿管囊肿,病变多位于脐下至膀胱顶之间,膀胱顶缘周围腹壁呈节段状的椭圆形(或不规则形)囊性(或混合性)包块,内部回声杂乱不均;诊断准确率为77.78%(7/9)。③脐尿管瘘,多呈管状或盲管状,低至无回声;诊断准确率为94.87% (37/39)。④脐尿管肿瘤1例,表现为脐下缘腹壁与膀胱顶部间不规则混合性包块,边界不清,内部少许细点状强回声,实性部分有较丰富的血流信号,血流阻力增高,膀胱壁局部稍厚,加压时包块不随肠管变化。结论 超声能够较准确地显示成人脐尿管病变的范围、特征及与周围组织的关系等,可为临床诊疗提供重要的影像学信息。 |

| Objective To explore the applicative value of ultrasonography in diagnosis of urachal lesions in adults. Methods Ultrasonographic findings of 87 adults confirmed by clinical diagnosis and pathology after operation were analyzed retrospectively. Results There were 4 kinds of ultrasonographic findings of urachal lesions. ①The ultrasonographic features of umbilical polyp inflammation or umbilical sinus showed hypoechogenicity or anechoic at the center of umbilicus or around it, and the diagnostic accuracy rate was 100% (38/38). ②The ultrasonographic features of allantoic cyst showed oval (or irregular) shape, cystic (or mixed) mass and inhomogeneous echo between umbilicus and the top of the bladder, and the diagnostic accuracy rate was 77.78% (7/9). ③The ultrasonographic features of urachal fistula showed tube or blind tube, low echo or echoless, and the diagnostic accuracy rate was 94.87% (37/39). ④The ultrasonographic features of urachus tumor showed irregular and mixed mass between margo inferior umbilicus and apex vesicae, no clear boundary line, a few strong echo, abundant blood flow signals in the solid parts, higher flow resistance, thicker wall in some part of the bladder, independent from bowels. Conclusion Ultrasonography can be used to show the range and characteristics of the lesions and the relationship with adjacent tissues clearly, and it is helpful for diagnosis and treatment of urachal lesions in adults. |
