目的 观察儿童肝脏间叶错构瘤(HMH)的CT影像学特点。方法 收集经术后病理证实的19例HMH患儿,术前均行CT检查,回顾性分析其临床及CT影像学特征。根据CT平扫图像显示的病变密度,将瘤灶分囊性、囊实性、实性3种类型。结果 19例HMH病灶均为巨大类圆形或长椭圆形,肿瘤最大径4~23 cm,CT定位诊断准确率为78.95%(15/19)。CT平扫12例表现为囊实性,4例为单纯实性,3例为多分隔囊性占位。1例囊实性占位内见点状钙化。增强后囊性成分无强化,实性成分轻、中度不均匀性强化。7例(3例囊实性和4例实性)病灶内见肝血管分支穿行。结论 HMH的CT平扫可表现为多分隔囊性、囊实性和实性,瘤灶实性成分强化模式多样。 |

Objective To observe the CT images of pediatric hepatic mesenchymal hamartoma (HMH) in children. Methods Nineteen patients with pathology confirmed HMH were collected, and all children underwent pre-operation CT scan. The clinical and CT imaging features were reviewed retrospectively. According to the lesion density of plain CT image, the tumor was divided into three types, i.e. multicystic masses, cystic-solid mixed masses and solid masses. Results Nineteen lesions were all in shape of round or long elliptic with 4—23 cm diameter, and 78.95% (15/19) were correctly positioned by CT, 12 diagnosed as cystic-solid HMH, 4 as solid HMH, and 3 as multiple separate cystic HMH. Calcification was found in 1 of cystic-solid HMH. The solid component in cystic-solid mixed and solid HMH and the septations in cystic HMH enhanced mildly or moderately. The cystic components could not been enhanced on enhanced CT. Minute branches of hepatic vessel were visible in 3 of cystic-solid mixed HMH and 4 of solid HMH. Conclusion HMH could showed multiseptate cystic, solid and cystic-solid mixed lesions, and diversity enhanced patterns in solid and cystic-solid mixed lesions on CT imagings. |
