目的 探讨采用实时剪切波弹性成像(SWE)检测急性缺血性脑卒中患者颈动脉纵向弹性模量的可行性。方法 选取急性缺血性脑卒中(AIS)患者179例(脑卒中组),其中大动脉粥样硬化性脑梗死(LAA)103例,腔隙性脑梗死(LAC)患者76例。另选取年龄、性别匹配的正常对照组168例。采用超声射频技术测量双侧颈动脉脉搏波传导速度(PWV)。SWE检测双侧颈动脉浅壁20个ROI纵向平均弹性模量的平均值(MEmean)、最大弹性模量的平均值(MEmax)和最小弹性模量的平均值(MEmin)。分析影响动脉僵硬度的相关因素。结果 ①与对照组相比,脑卒中患者颈动脉僵硬度参数PWV增大、颈动脉浅壁纵向弹性模量参数MEmean和MEmax增大(P均<0.05)。②所有受试者MEmean和MEmax与年龄、收缩压、颈动脉管径、PWV和低密度脂蛋白胆固醇呈正相关,与总胆固醇呈负相关(P均<0.05)。结论 采用SWE可通过检测颈动脉纵向弹性模量评价动脉僵硬度。 |

Objective To evaluate the carotid longitudinal elastic modulus in patients with acute ischemic stroke (AIS) using real-time shear wave elastography (SWE). Methods In Total, 179 cases (AIS group) with AIS including 103 cases with large artery atherosclerosis (LAA) and 76 cases with lacunar infarction (LAC); and 168 age- and sex-matched health volunteers(control group) were collected. The pulse wave velocity (PWV) of bilateral carotids was measured by radio frequency ultrasound technology. In the 20 ROI of superficial walls of bilateral carotids, the average values of longitudinal average elastic modulus (MEmean), the maximum elastic modulus (MEmax) and minimum elastic modulus (MEmin) were analyzed by real-time SWE, espectively. The relative factors of arterial stiffness were analyzed. Results ①Compared with the control group, the PWV and the MEmax and MEmean of carotid artery in AIS group increased (all P<0.05). ②MEmean and MEmax were positively related to age, systolic blood pressure, diameter of common carotid arterial, PWV, low-density lipoprotein, but were negatively correlated with low-density lipoprotein (all P<0.05). Conclusion The carotid longitudinal modulus of elasticity could be analyzed by SWE and it could be used as an indicator for arterial stiffness. |
