      目的 探讨体素内不相干运动成像(IVIM)在原发性直肠癌术前检查中的价值.方法 采用3.0T MR扫描仪IVIM序列(b=0、25、50、75、100、150、200、400、600、800、1000、1200、1500、2000 s/mm2)和T2cube序列获得62例直肠癌患者术前的影像资料.测量和比较正常肠壁与肿块、不同分化程度直肠癌的ADC、D*、D和f值;评价IVIM中不同b值的DWI图像的SNR和CNR.结果 直肠癌肿块的ADC、D、D*和f值均明显小于正常肠壁(P均 <0.05);中、低分化直肠癌ADC值差异有统计学意义(F=31.92,P=0.01),高、中、低分化直肠癌D值差异有统计学意义(F=16.79,P=0.03);b=1000 s/mm2时的DWI图像质量最好,肿块与正常肠壁显示最佳.结论 IVIM图像可提供肿瘤ADC、D、D*和f值的信息,可在一定程度上反映肿瘤的病理生理状态,其中D值较ADC值更能够准确地反映直肠癌组织间分化程度的差别,在b=1000 s/mm2时图像质量最佳,有利于发现直肠癌病灶和了解术前肿瘤的分化程度.

      Objective To evaluate the clinical value of intravoxel incoherent motion imaging (IVIM) in diagnosis of rectal carcinoma. Methods Sixty-two patients with rectal carcinoma underwent IVIM (b=0, 25, 50, 75, 100, 150, 200, 400, 600, 800, 1000, 1200, 1500 and 2000 s/mm2) at 3.0T MR scanner were enrolled. The ADC, D, D* and f values were measured and calculated in IVIM sequence; all patients were divided into high, medium and low differentiation, and the ADC, D, D* and f values were compared respectively. The different b values images of IVIM were evaluated by SNR and CNR. Results The ADC, D, D* and f values of rectal tumor showed significant reduction compared with those of normal rectal wall (all P <0.05); the difference of ADC value in medium differentiation and low differentiation were statistically significant (F=31.92, P=0.01); the difference of D value in high, medium and low differentiation were statistically significant (all P <0.05), but of D* and f value were not statistically significant (F=16.79, P=0.03). The best b value of diffusion weighted images in IVIM sequence were 1000 s/mm2. Conclusion IVIM can reveal ADC, D, D* and f values of rectal carcinoma, which is useful for demonstrating the pathophysiology process of rectal cancer. D value is more significant than ADC value in histology differentiation. The optimal images are shown in b=1000 s/mm2.
