       目的 探讨18氟脱氧葡萄糖(18F-FDG)PET/CT对甲状腺结节的诊断价值。方法 回顾性分析接受18F-FDG PET/CT检查并有明确病理结果的甲状腺结节患者79例(94个结节)。对比甲状腺恶性与良性结节的最大标准化摄取值(SUVmax),并进行ROC曲线分析,计算18F-FDG PET/CT诊断甲状腺良恶性结节的灵敏度、特异度、准确率、阳性预测值及阴性预测值。结果 共94个结节,恶性、良性结节分别为53个(53/94,56.38%)和41个(41/94,43.62%)。甲状腺恶性、良性结节的SUVmax分别为7.15±5.09和2.58±1.19,差异有统计学意义(Z=5.33,P<0.01)。SUVmax诊断恶性甲状腺结节的ROC曲线下面积为0.82(95%CI:0.74~0.91,P<0.01),SUVmax界值为4.50时,灵敏度与特异度之和最大,为1.56。18F-FDG PET/CT诊断的灵敏度、特异度、准确率、阳性预测值及阴性预测值分别为79.25%(42/53)、80.49%(33/41)、79.79%(75/94)、84.00%(42/50)、75.00%(33/44)。结论 18F-FDG PET/CT可以判断甲状腺良、恶性结节,SUVmax界值应为4.50。

      Objective To investigate the value of 18F-FDG PET/CT in diagnosis of thyroid nodules. Methods All 79 patients with 94 thyroid nodules confirmed by histopathology underwent 18F-FDG PET/CT. Maximum standardized uptake value (SUVmax) of benign and malignant thyroid nodules were compared, and ROC curve of SUVmax in diagnosis of malignant thyroid nodules were analyzed. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of 18F-FDG PET/CT in diagnosis of malignant and benign thyroid nodules were calculated. Results There were 53 malignant (53/94, 56.38%) and 41 benign (41/94, 43.62%) thyroid nodules in 94 nodules. The SUVmax of malignant and benign thyroid nodules were 7.15±5.09 and 2.58±1.19, respectively (Z=5.33,P<0.01). The ROC area under curve for SUVmax in diagnosis of malignant thyroid nodules was 0.82 (95%CI: 0.74-0.91, P<0.01). When the SUVmax was 4.50, the sum of the sensitivity and the specificity was max of 1.56. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of 18F-FDG PET/CT in diagnosis of malignant thyroid nodules were 79.25% (42/53), 80.49% (33/41), 79.79% (75/94), 84.00% (42/50), 75.00% (33/44), respectively. Conclusion 18F-FDG PET/CT can differentially diagnosis of malignant and benign thyroid nodules, the SUVmax boundary value should be 4.5.
