目的 探讨感音神经性耳聋患儿内耳畸形发生率及其超高分辨率CT表现。方法 回顾性分析449例(共计861耳)感音神经性耳聋患儿颞骨超高分辨率CT表现,计算内耳畸形的发生率。结果 内耳畸形249耳,发生率为28.92%(249/861)。耳蜗畸形163耳(163/861,18.93%),其中蜗轴发育不良107耳(107/861,12.43%);前庭畸形136耳(136/861,15.80%),其中前庭扩大131耳(131/861,15.21%);半规管畸形153耳(153/861,17.77%);前庭导水管扩大165耳(165/861,19.16%);内听道狭窄43耳(43/861,4.99%)。内耳单一结构畸形52耳(52/861,6.04%);内耳多结构合并畸形197耳(197/861,22.88%),其中耳蜗-前庭-半规管-前庭导水管合并畸形69耳(69/861,8.01%)。结论 前庭导水管扩大、蜗轴发育不良、前庭扩大和内听道狭窄为较常见的内耳畸形;内耳多个结构同时合并畸形较单一结构畸形常见。 |

Objective To evaluate the findings and incidence of inner ear malformations in children with sensorineural hearing loss by ultra high resolution CT. Methods The findings of the temporal bone in 449 cases (861 ears) were retrospectively analysed with ultra high resolution CT, and the incidence of inner ear malformations were calculated. Results Inner ear malformations were detected in 249 ears, accounting for 28.92% (249/861) of the children with sensorineural hearing loss. Cochlear malformation was identified in 163 ears (163/861, 18.93%), with modiolus dysplasia in 107 ears (107/861, 12.43%); Vestibular malformation was displayed in 136 ears (136/861, 15.80%), with dilated vestibule in 131 ears (131/861, 15.21%); Semicircular canal malformation was shown in 153 ears (153/861, 17.77%); Large vestibular aqueduct was seen in 165 ears (165/861, 19.16%); Narrow internal auditory canal was found in 43 ears (43/861, 4.99%). Single structure of inner ear was involved in 52 ears (52/861, 6.04%); Abnormalities of multiple structures of the inner ear were simultaneously found in 197 ears (197/861, 22.88%), with cochlea-vestibule-semicircular canal-vestibular aqueduct anomaly in 69 ears (69/861, 8.01%). Conclusion Large vestibular aqueduct, modiolus dysplasia, dilated vestibule and narrow internal auditory canal are common inner ear malformations, and abnormalities of multiple structures of the inner ear is more common than single structure abnormality. |
