目的 应用二维斑点追踪成像(2D-STI)技术评价肺动脉高压(PAH)大鼠左心室多维形变。方法 将健康雄性SD大鼠50只,分为实验组(3、4、5、6周亚组)和对照组。实验组均按60 mg/kg体质量皮下注射1%野百合碱,分别于建模3、4、5、6周后行超声心动图检查。对照组等比例注射生理盐水,24 h后行超声心动图检查。常规二维超声测量左、右心功能参数,采用2D-STI测量左心室纵向、圆周及径向整体收缩期应变峰值(GSL、GSC和GSR)、收缩期应变率峰值(GSrL-S、GSrC-S和GSrR-S),并分析其与右心收缩功能指标的相关性。结果 实验组大鼠右心功能逐步衰退,右心室壁厚度(RVWT)及右心室肥厚指数(RVHI)随周龄增大,右心室面积变化率(RVFAC)及三尖瓣瓣环收缩期位移(TAPSE)逐渐减低(P均<0.05)。左心室射血分数和缩短率逐渐减低,其中5周及6周亚组与其他亚组和对照比较差异有统计学意义(P均<0.05);与对照组相比,3周、4周、5周、6周亚组GSL、GSC和GSR均减低;GSrL-S、GSrC-S和GSrR-S亦减低,其中GSrL-S在3周时即表现出与对照组间的差异(P均<0.05)。GSL、GSC、GSR、GSrL-S、GSrC-S和GSrR-S与RVFAC均呈正相关(P均<0.05)。结论 2D-STI可观察PAH右心受损大鼠左心室的功能变化。 |

Objective To investigate the left ventricular multi-dimensional motion of pulmonary arterial hypertension (PAH) rat models with two-dimensional speckle tracking imaging (2D-STI). Methods Fifty healthy male SD rats were randomly divided into experimental group (including 3, 4, 5, 6 weeks subgroups) and control group. Each rat of experimental group got an intraperitoneal injection of 1% monocrotaline with the dose of 60 mg/kg, and then accepted the echocardiography at the end of 3rd, 4th, 5th and 6th week after injection. The rats of control group were injected with the same amount of normal saline and accepted the echocardiography after 24 h. General parameters of left and right heart were measured. The peak value of global longitudinal, circumferential and radial strain (GSL, GSC and GSR) and systolic strain rate (GSrL-S, GSrC-S and GSrR-S) were measured with 2D-STI. The correlation between the 2D-STI parameters and right ventricular systolic function was analyzed. Results After monocrotaline-injection, the right heart function of experimental rats decreased, right ventricular wall thickness and right ventricular hypertension index enlarged gradually, while right ventricular fractional area change and tricuspid annular plane systolic excursion decreased week by week (P<0.05). Meanwhile, left ventricular ejection fraction and fractional shortening decreased gradually. While 5 weeks and 6 weeks subgroups showed significant difference compared with control group and other experimental groups (all P<0.05). Using 2D-STI, GSL, GSC, GSR of 3, 4, 5 and 6 weeks subgroups decreased comparied with control group. GSrL-S,GSrC-S and GSrR-S decreased, too. Compared with control group, GSrL-S showed significant difference even in 3 weeks subgroup (P<0.05). GSL, GSC, GSR, GSrL-S, GSrC-S and GSrR-S all had positive correlations with RVFAC (all P<0.05). Conclusion 2D-STI can assess the dynamic changes of multi-dimensional motion of PAH rat models. |
