目的 分析影响超声引导下经皮穿刺活检胸部病变并发症的影响因素。方法 收集因胸部病变行超声导引下穿刺活检术的178例患者资料,采用单因素和多因素Logistic回归分析方法,分析患者相关因素(性别、年龄)、病灶相关因素(病灶的大小、部位、性质、血供、病灶内有无坏死、病理类型)、操作相关因素(穿刺胸膜次数)对穿刺活检并发症发生率的影响。结果 178例患者均穿刺取材成功,穿刺诊断正确率为83.15%(148/178),并发症发生率为7.30%(13/178),其中出血发生率为6.18%(11/178);单因素分析结果显示病灶的大小、穿刺胸膜次数对并发症的发生率均有影响(P<0.05);多因素分析结果显示穿刺胸膜次数为并发症发生率的危险因素(β=1.683),病灶的大小为保护因素(β=-0.999)。结论 病灶的大小、胸膜穿刺次数是影响超声引导下穿刺活检胸部病变并发症发生率的独立影响因素,其中胸膜穿刺次数为危险因素,而病灶的大小为保护因素。 |

Objective To investigate the factors which influence the incidence of complications in performing ultrasound-guided percutaneous biopsy of chest lesions. Methods The data of 178 patients who underwent ultrasound-guided percutaneous biopsy in chest lesions were enrolled. The univariate and multivariate Logistic retrospective analysis were performed to study the influence of the patient-related factors (sex, age), lesion-related factors (lesion size, location, blood supply, with or without necrosis, nature of the lesion, pathological type), and procedure factors (number of pleural passed) on the incidence of complications by ultrasound-guided. Results Puncture was succeed in all 178 cases. The diagnostic accuracy rate of ultrasound-guided percutaneous biopsy was 83.15% (148/178); the complication rate was 7.30% (13/178), among them the frequency of hemorrhage were 6.18% (11/178). The results in single factor analysis showed that the size of the lesion, number of pleural passed could effect on the incidence of complications (P<0.05); the results in the multi-factor analysis showed that the number of pleural passed was a risk factor (β=1.683), while the size of the lesion was a protective factor (β=-0.999). Conclusion The size of the lesion and the number of pleural passed are independent factors which influence the incidence of complications in performing ultrasound-guided percutaneous biopsy of chest lesions, and the number of pleural passed is a risk factor, while the size of the lesion is a protective factor. |
