目的 应用二维斑点超声追踪显像技术评价孕期妇女左心房功能变化。方法 对25名孕妇和24名年龄匹配的未孕女性(对照组)进行超声心动图检查。所有孕妇产后第6个月再次行超声检查。通过双平面Simpson's法获得左心房最大容积、左心房主动收缩前容积、左心房最小容积。通过计算获得左心房充盈容积、左心房扩张指数、左心房射血分数,被动射血容积、被动射血指数、主动射血容积、主动射血指数等指标。同时应用二维斑点超声追踪显像技术测量左心房应变及应变率。结果 与对照组相比,产前组左心房储蓄功能及辅助泵增强,而左心房管道功能减低(P<0.05)。而对照组和产后组之间的左心房功能指标的差异均无统计学意义(P>0.05)。结论 孕期左心房管道功能受损,储蓄功能及辅助泵功能均增强。这种变化随着分娩的结束而得到恢复。二维斑点超声追踪显像技术可以准确评价孕期左心房功能变化。 |

Objective To evaluate the changes of left atrial (LA) function during pregnancy using two-dimensional speckle tracking echocardiography (2DSTE). Methods Totally 25 pregnant and 24 nulliparous (control group) women were enrolled in this study. All pregnant women were followed up postpartum in sixth-month. The LA maximum volume, LA minimal volume and LA presytolic volume were obtained using biplane modified Simpson's method. LA filling volume, LA expansion index, LA ejection fraction, passive volume, passive emptying index, active volume, and active emptying index were calculated. LA longitudinal systolic strain (SS), systolic strain rate (s-SR), early diastolic strain rate (e-SR), and late diastolic strain rate (a-SR) were obtained by 2DSTE. Results Compared to the control group, the reservoir function was increased in pregnant patients (P<0.05), conduit function was decreased in pregnant patients (P<0.05), and booster pump function was increased in pregnant patients (P<0.05). There was no statistically significant difference between the control group and postpartum group. Conclusion LA reservoir and booster pump function were improved, while conduit function was impaired during pregnancy. The changes were reversible. 2DSTE can be used to assess LA function during pregnancy accurately. |
