目的 探讨利用DWI ADC值对AS患者骶髂关节炎症活动进行量化评价的可行性。方法 将22例AS患者根据AS病情活动指数(BASDAI)评分分为静止组(<4分,n=8)及活动组(≥4分,n=14)。另选18名健康志愿者作为正常对照组。对3组均行骶髂关节MR检查,获得各组骶髂关节骨性关节面下及L5椎体骨髓的ADC值(ADCSIJ值、ADCL5值)及骶髂关节的相对ADC值(rADCSIJ值)。对静止组及活动组于MR检查前行C反应蛋白(CRP)及血沉(ESR)检查,并进行统计学分析。结果 静止组与正常对照组间ADCSIJ值、rADCSIJ值差异均无统计学意义(P均>0.05)。活动组与静止组及正常对照组间ADCSIJ值、rADCSIJ值差异均有统计学意义(P均<0.05)。3组间ADCL5值差异均无统计学意义。AS患者ADCSIJ值、rADCSIJ值与临床BASDAI评分、ESR及CRP均呈正相关(P均<0.05)。结论 DWI ADC值有助于临床辅助诊断活动期AS并判断骶髂关节炎症活动程度。 |

Objective To investigate the feasibility of quantitative evaluating sacroiliac joint inflammatory activities in patients with ankylosing spondylitis (AS) using DWI ADC value. Methods Totally 22 AS patients were divided into static group (<4 points, n=8) and active group (≥4 points, n=14) according to Bath ankylosing spondylitis disease activity index (BASDAI). And 18 healthy volunteers were enrolled as control group. The ADC value of sacroiliac joint under joint surface (ADCSIJ ), the bone marrow ADC value of L5 vertebral (ADCL5) and the relative ADC value of sacroiliac joint (rADCSIJ) were obtained by MR scan. Examinations of C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) were performed before MR scanning. Moreover, statistical analysis was done. Results There was no statistically significant difference of ADCSIJ and rADCSIJ values between static group and control group (P>0.05). There was statistically difference of ADCSIJ and rADCSIJ values between active group and static group, as well as those between active group and control group (both P<0.05). No statistical difference of ADCL5 value was found among three groups. The ADCSIJ and rADCSIJ values of AS patients correlated positively with BASDAI scores, ESR and CRP (all P<0.05). Conclusion DWI ADC value is helpful to diagnose activity AS and determine the degree of sacroiliac joint inflammatory activity. |
