      目的 探讨磁共振T1ρ成像检测兔膝关节软骨早期退变的可行性。方法 将32只实验兔随机分组;其中24只(实验组)随机分为A~D组,每组6只,均予左侧膝关节腔内注射木瓜蛋白酶,建立关节软骨早期退变的模型;余8只作为空白对照组(E组),随机分为EA、EB、EC、ED亚组,每组2只,分别做为A、B、C、D组的空白对照。于末次注药后1、2、3、4周分别对A~D组(对EA、EB、EC、ED亚组进行同期对照成像)行双膝常规T2W、3D-FS-SPGR和T1ρ扫描后处死并取双膝关节股骨髁软骨做组织学检查。测量并分析兔股骨髁软骨T1ρ值组内及组间的差异,并与组织学结果对照。结果 实验组处理侧(左侧)股骨髁软骨T1ρ值均明显高于对照侧(P<0.05);实验组处理侧股骨髁软骨T1ρ值随时间推移逐渐升高(P<0.05);T1ρ伪彩图显示实验组处理侧软骨呈浅绿色向深绿色的渐进性改变;组织学检查显示实验组处理侧股骨髁软骨发生早期退变,软骨内PG含量明显下降。结论 T1ρ值能够敏感地量化反映关节软骨退变早期、形态学改变之前的蛋白多糖变化。T1ρ成像可能为临床检测关节软骨早期退变提供影像学依据。
英文摘要:
      Objective To explore the feasibility of T1ρ magnetic resonance imaging for detection of early articular cartilage degeneration of rabbit models. Methods A total of 24 rabbits were randomly divided into four experimental groups (group A, B, C, D, each n=6) to establish articular cartilage models at different stage of early degeneration by injecting papain into the left knee joint cavity, and 8 rabbits were considered as blank control (group E), and then randomized into four subgroups (EA, EB, EC, ED, each n=2). The T1ρ, 3D-FS-SPGR and T2W images of the bilateral knee joints were obtained for 32 rabbits using a 3.0T MRI. Group A, B, C, D were scanned in sequence after one to four week from the last injection, and EA, EB, EC, ED underwent the same sequence scanning during the period. A total of 32 rabbits were executed after scanning and the femoral condyle cartilage were histologically examined. T1ρ values of the femoral condyle cartilage were measured and statistically analyzed, and compared with the histologic results. Results T1ρ values of the experimental side (left side) were significantly higher than that of the control side (right side, P<0.05). T1ρ values of the experimental side increased gradually with the passage of time (P<0.05). T1ρ pseudo color map of the experimental side showed a tale of gradual change from light green to dark green, histological examination demonstrated the proteoglycan content of the experimental side decreased, and indicated the occurrence of early degeneration. Conclusion T1ρ values can sensitively and quantitatively reflect the changes of proteoglycans within articular cartilage before the morphologic alterations, therefore T1ρ could potentially act as a clinical tool to assess the prophase cartilage degeneration.
