      目的 应用能谱CT基物质成像技术评价卵巢切除术后腰椎钙(水)密度变化特征。方法 对29例50~60岁卵巢切除术后患者(卵巢切除组,手术≤1年11例、手术>1年18例)及31例同年龄段绝经后患者(对照组)进行腹部能谱CT检查。由2名放射科医师测量L1~3椎体中心层面的钙(水)密度,分析2名医师测量结果的一致性、腰椎不同位置钙(水)密度的组内差异及两组间钙(水)密度差异。结果 2名医师的测量结果具有一致性(组内相关系数0.97~0.99);卵巢切除术后组、对照组的L1、2、3椎体相邻椎体间的钙(水)密度差异无统计学意义(P均>0.05);卵巢切除术后≤1年和>1年患者腰椎的钙(水)密度间差异无统计学意义(P>0.05),但均低于对照组,差异有统计学意义(P<0.05);卵巢切除术后患者腰椎的钙(水)密度较对照组下降约15%。结论 CT能谱成像对卵巢切除术后腰椎钙(水)密度监测具有一定意义,卵巢切除可使绝经后女性腰椎的钙(水)密度进一步下降。
英文摘要:
      Objective To assess lumbar calcium (water) density with material decomposition images of spectral CT in patients with ovariectomy. Methods Twenty-nine patients with ovariectomy (ovarieclomy group, 11 patients with ovariectomy ≤1 year, 18 patients with ovariectomy >1 year) and 31 postmenopausal patients (control group) were included. All patients underwent abdominal spectral CT examination. Two experienced radiologists independently measured the density calcium-water and interobserver agreement were analyzed. Mean DCa(Wa) values of the two groups and the mean DCa(Wa) values of adjacent lumbar vertebra were compared. Results DCa(Wa) values of interobserver were perfect agreement (ICC 0.97—0.99), the DCa(Wa) between L1 and L2, L2 and L3 had no significant difference in the two groups (P>0.05).The DCa(Wa) between patients with ovariectomy ≤1 year and the patients with ovariectomy >1 year had no significant difference (P>0.05), but all were lower than control group (P<0.05), the DCa(Wa) of L1—3 vertebral body showed significant drops in patients with ovariectomy. Conclusion The calcium and water-based material decomposition images of spectral CT are useful to evaluate the lumbar calcium (water) density changes in the patients with ovariectomy, and ovariectomy may further reduce lumbar calcium (water) density.
