目的 应用多层螺旋CT血管成像(MSCTA)探讨寰椎沟桥的涵义、分型及其对椎动脉影响。方法 对头颈部MSCTA检查发现的92例沟桥原始数据采用VRT技术重建,观察沟桥的表现及其与椎动脉的关系。结果 本组92例沟桥(133侧)中,单侧51例,双侧41例;单纯沟桥112侧,复合桥21侧;单纯沟桥中,后桥(PP)、侧桥(PL)、边桥(PB)的检出率分别为68.75%(77/112)、19.64%(22/112)、11.61%(13/112),而半环型(HR)、断环型(DR)、全环型(WR)的检出率分别为51.79%(58/112)、25.89%(29/112)、22.32%(25/112)。对椎动脉造成影响23侧,其中受压变形15侧、长期痉挛性狭窄8侧、受激惹痉挛4侧、椎动脉走行异常3侧,部分为多种影响同时发生。结论 应用MSCTA可观察沟桥形态及其对椎动脉的影响,有助于对沟桥的临床意义进行研究探讨。 |

Objective To explore the meaning, classification and influence to vertebral artery of the ponticulus of Atlas by multi-slice spiral computed tomography angiography (MSCTA). Methods Totally 92 patients with ponticulus were collected. All the patients underwent the examine of intracranial and cervical and volume rendering technique (VRT), and the manifestation and relation to vertebral artery were observed. Results A total of 133 sides ponticulus were detected in 92 cases, including unilateral in 51 cases, bilateral in 41 cases, and the simple type ponticulus were 112 sides, the ponticulus compound (PC) were 21 sides. In the simple type ponticulus, the detection rate of the ponticulus posterior (PP), ponticulus lateralis (PL), ponticulus borderland (PB), was 68.75% (77/112), 19.64% (22/112), 11.61% (13/112), and the detection rate of the half ring (HR), disconnected ring (DR), whole ring (WR) was 51.79% (58/112), 25.89% (29/112), 22.32% (25/112). Totally 23 sides vertebral artery were caused by ponticulus, including compressive deformation 15 sides, long-term spastic constriction 8 sides, irritation spasms 4 sides, vertebral artery courser anomaly 3 sides, and some of them with variety effects at the same time. Conclusion MSCTA can be used to observe the shape of the ponticulus and the effect on vertebral artery, and it is helpful for the research on clinical significance of the ponticulus. |
