目的 观察原发气道唾液腺型肿瘤 的CT表现。方法 回顾性分析经病理证实的22例气道ACC和23例MEC患者的CT表现。结果 病变部位:ACC位于气管及主、叶支气管,以气管最多(12/22,54.55%),MEC位于段及以上各气道,以主支气管最多(9/23,39.13%)。ACC形态:气管腔内外肿物10例,气道壁弥漫增厚5例,肿物截断主支气管者4例,腔内宽基底肿物3例。MEC形态:气道腔内肿物20例,腔内外肿物2例,肿物截断主支气管者1例。密度:ACC平扫CT值24~35 HU、低于邻近胸壁肌肉,多轻度强化(14/19,73.68%);MEC平扫CT值40~60 HU,4例伴少量钙化,多明显强化(16/20,80.00%)。结论 气道唾液腺型肿瘤CT表现具一定特点,CT影像能较准确的显示病变部位、形态及其与周围组织的关系。 |

Objective To observe the CT features of primary airway salivary gland-type neoplasms, including adenoid cystic carcinoma (ACC) and mucoepidermoid carcinoma (MEC). Methods The CT findings and clinical data of 22 patients with ACC and 23 patients with MEC in airway confirmed by pathology were analyzed retrospectively. Results ①Location, ACC located in trachea, main or lobar bronchus, and most of them (12/22, 54.55%) located in trachea. MEC located in segmental and above segmental bronchus, mainly in main bronchus (9/23, 39.13%). ②Shape, ten cases of ACC presented intra-and-extraluminal lesions located in trachea, 5 cases of ACC presented diffuse wall thickening lesions, 4 cases of ACC showed tumor which cut off the main bronchus, and 3 cases of ACC presented intra-luminal wide base lesions. Twenty cases of MEC showed intra-luminal lesions, 2 cases of MEC showed intra-and-extraluminal infiltrated lesions, 1 case of MEC showed tumor which cut off the main bronchus. ③Density, the CT values of ACC (24—35 HU) were lower than that of adjacent chest wall muscle on plain scan and most of them slightly enhanced on enhanced CT (14/19, 73.68%). The CT values of MEC was 40—60 HU on plain scan, and the calcification were observed in 4 cases, and most MEC (16/20, 80.00%) showed obvious enhancement on enhanced CT. Conclusion The CT manifestations of airway salivary gland-type neoplasms have certain characteristics, CT imaging can provide precise information about location, shape and the relationship with the surrounding tissue. |
