目的 观察粒细胞肉瘤(GS)的影像学特征。方法 回顾性分析27例经病理证实的GS患者的影像学资料,11例接受CT检查,其中4例接受增强扫描;9例接受MR平扫,其中7例接受动态增强扫描;6例接受超声检查;4例接受FDG PET/CT检查。结果 27例GS中,14例为白血病性GS,5例孤立性GS,8例未知分型;15例多发,12例单发,共计40个病灶,其中软组织GS 13个,淋巴结GS 12个,胸腹膜及胃肠道GS 5个,生殖系统GS 4个,中枢神经系统GS 3个,骨及骨膜GS 3个。40个病灶中,29个形态不规则,27个边界不清;CT扫描11例,呈等或略低密度,病灶中心CT值均较外周稍低,4例增强后均明显强化;MR扫描9例,DWI显示均明显弥散受限,8例呈等T1等T2信号,T2WI上病灶中心信号强度较外周稍低,7例动态增强中6例呈速升缓降型。PET/CT检查4例,病灶均代谢活跃,2例见全身骨髓弥漫性代谢活跃;超声检查6例,均呈低回声,2例可见血流信号。结论 GS的影像学表现具有一定特征性,结合临床病史有助于做出初步诊断。 |

Objective To observe the imaging features of granulocytic sarcoma (GS). Methods A total of 27 patients with GS confirmed by pathology were analyzed retrospectively, and the CT (n=11, 4 patients underwent enhanced scan), MRI (n=9, 7 patients underwent enhanced scan), ultrasound (n=6) and FDG PET/CT (n=4) features were observed. Results Among 27 cases of GS, 14 cases were leukemic GS, 5 cases were isolated GS, 8 cases with unknown typing, 15 cases appeared as multiple lesions and 12 cases with isolated lesion, all of the 40 sites showed as follows, soft tissues (n=13), lymph nodes (n=12), pleura/peritoneum/gastrointestinal system (n=5), genitourinary system (n=4), central nervous system (n=3) and bone/periosteum (n=3), 29 of 40 sites showed irregular shapes and 27 cases appeared as obscure boundarys. Eleven cases of CT images showed iso- or hypo-density features, with CT values of peripheral components higher than that of center, 4 cases underwent contrast-enhanced CT scan and showed obviously enhancement. Nine cases underwent MRI scan and DWI showed distinctly diffusion restricted, 8 cases showed iso-intensity on both T1WI and T2WI, the peripheral with relatively hyper-intensity than center on T2WI. Seven cases were performed dynamic-enhanced MRI scan, 6 cases presented as the type of "speedily up slowly down". Four cases underwent FDG PET/CT scan and showed metabolically active lesions, 2 cases appeared metabolic activity of the whole body bone marrow. Six cases presented as hypo-echo masses on ultrasound, 2 cases with blood flow signals. Conclusion The imaging characteristics of GS share some common features, and the combination with clinical manifestation are helpful for primary diagnosis of GS. |
