| 目的 探讨椎动脉发育异常伴胚胎型大脑后动脉(FTP)与后循环缺血(PCI)之间的关系。方法 回顾性分析接受头颅容积CT数字减影血管成像(VCTDSA)检查的218例PCI患者(PCI组)和110名无前后循环缺血临床表现的健康志愿者(对照组)。于VR图上观察有无孤立性椎动脉(IVH)、椎动脉发育不良(HVA)及FTP,并测量双侧椎动脉颅内段起始处管径。对椎动脉发育异常合并FTP与PCI进行相关性分析。结果 ①对照组中,IVH占1.82%(2/110),HVA占17.27%(19/110),FTP占7.27%(8/110);其中椎动脉发育异常伴FTP占0.91%(1/110);②PCI组中,IVH占10.55%(23/218),HVA占49.08%(107/218);FTP占28.90%(63/218);椎动脉发育异常伴FTP占25.23%(55/218);③对照组与PCI组间椎动脉发育异常伴FTP的检出率差异有统计学意义(χ2=35.56,P<0.001);④椎动脉发育异常伴FTP与PCI具有相关性(r=0.31,P<0.001)。结论 椎动脉发育异常伴FTP可能引起PCI。 |

| Objective To explore the relationship between abnormal development of vertebral artery with fetal posterior cerebral artery (FTP) and posterior circulation ischemia (PCI). Methods A total of 218 patients with PCI (PCI group) and 110 subjects (control group) were concomitantly enrolled and underwent volume computed tomographic digital subtraction angiography (VCTDSA). The VR images were analyzed retrospectively and the hypoplastic vertebral artery (HVA), isolated vertebral artery (IVA) and FTP were observed, the diameters of the vertebral artery were measured, and then the correlation between abnormal development of vertebral artery with FTP and posterior circulation ischemia (PCI) were a analyzed. Results ①In control group, IVA, HVA, FTP and abnormal development of vertebral artery with FTP was 1.82% (2/110), 17.27% (19/110), 7.27% (8/110) and 0.91% (1/110), respectively. ②In PCI group, IVA, HVA, FTP and abnormal development of vertebral artery with FTP was 10.55% (23/218), 49.08% (107/218), 28.90% (63/218) and 25.23% (55/218), respectively. ③The significant differences of the abnormal development of vertebral artery with FTP were found between PCI group and control group (χ2=35.56, P<0.001). ④PCI was correlation with the abnormal development of vertebral artery with FTP (r=0.31, P<0.001). Conclusion The abnormal development of vertebral artery with FTP may lead to PCI. |
