目的 探讨CEUS时间-强度曲线(TIC)鉴别高、低级别膀胱尿路上皮癌的应用价值。方法 选取膀胱尿路上皮癌患者62例,根据2004年WHO膀胱癌病理分级标准,分为低级别组40例和高级别组22例。获取两组上升时间(RT)、峰值强度(PI)、平均渡越时间(MTT)、半洗漱时间(TPH)、达峰时间(TP)、上升斜率(WIS)、半降支斜率(DS)等定量参数,采用QLab软件分析膀胱尿路上皮癌CEUS特征。结果 两组膀胱尿路上皮癌肿块的形态、数目、内部回声、基底部情况、彩色血流比较,差异均无统计学意义(P均>0.05);高级别组TPH明显高于低级别组,DS低于低级别组(TPH:t=-4.17,P<0.01;DS:t=2.77,P=0.01),RT、PI、MTT、TP、WIS比较,差异均无统计学意义(P>0.05)。鉴别高、低级别膀胱尿路上皮癌的最佳截点TPH为58.06 s,DS为0.20 dB/s,曲线下面积均为0.79。结论 CEUS TIC曲线量化参数鉴别高、低级别膀胱尿路上皮癌具有一定的应用价值。 |

Objective To explore the application value of CEUS combined with time-intensity curve (TIC) on identify high and low grade bladder urothelial carcinoma. Methods Sixty-two patients with bladder urothelial carcinoma were selected and divided into low grade group (n=40) and high grade group (n=22) according to pathologic examination classification criterion of WHO 2004. TIC parameters included rise time (RT), peak intensiy (PI), mean transit time (MTT), time from peak to one half (TPH), time to peak (TP), wash in slope (WIS), semi-descending slope (DS) were acquired and CEUS characteristics were analyzed by QLab software. Results There were no statistically significantly differences in morphology, number, internal echo, basilar part and color flow of the tumors between two groups; the TPH was higher and DS was lower in high grade group than that in low grade group (TPH: t=-4.17, P<0.01; DS: t=2.77, P=0.01), but RT, PI, MTT, TP, WIS were no statistically significant between two groups (all P>0.05). The best cut-off point as identify high and low grade bladder urothelial carcinoma of TPH and DS were 58.06 s and 0.02 dB/s, and the area under the curve were 0.79. Conclusion There is certain differential diagnosis value of CEUS TIC quantization parameters in the high and low grade bladder urothelial carcinoma. |
