目的 探讨不同程度骨损伤状态下腘肌及腘肌腱(PMT)损伤后的MRI影像特征。方法 回顾性分析经关节镜或手术证实的33例PMT损伤患者,对全部病例均行膝关节外旋15° MR检查,28例行X线检查,15例行CT检查。将不同程度骨损伤时PMT的表现分为4型,观察其MRI表现。结果 Ⅰ型单纯腘肌、腘肌腱损伤(不伴有膝关节骨质异常)10例,3例MRI显示单纯性PMT内T2WI高信号改变;7例为PMT内T2WI不均一高信号改变伴PMT部分撕裂。Ⅱ型PMT损伤伴腓骨近端骨折4例,MRI显示为PMT内部T2WI不均一高信号改变,2例显示腘肌斜行撕裂,2例PMT多发撕裂。Ⅲ型PMT损伤伴胫骨近端骨折12例,1例为单纯性T2WI高信号改变,11例显示为T2WI不均一高信号改变,同时伴PMT多节段撕裂。Ⅳ型PMT损伤伴胫腓骨近端多发骨折7例,7例均显示PMT内T2WI高信号改变,同时伴PMT多节段撕裂。结论 PMT损伤的MRI影像特征表现为T2WI不均一高信号改变,同时多伴PMT撕裂。 |

Objective To evaluate the MRI features of popliteus muscle and tendon (PMT) injury in different bone changes in knee on MRI findings. Methods The MRI manifestations of 33 cases with PMT injury proved by arthroscopy and surgery was retrospectively analyzed. Imaging examinations were performed in MRI in all cases with knee external rotation 15°, radiography in 28 cases and multi-row CT in 15 cases. All cases with PMT injuries were classified as 4 types according to bone changes, which were observed with MRI. Results Type Ⅰ: Bone was normal in 10 cases. PMT on MRI showed hyperintense signal changes only on T2WI in 3 cases and multi-segment disruptions with heterogeneous high signal on T2WI in 7 cases. Type Ⅱ: With fracture in proximal fibula in 4 cases. Poplieus muscle showed oblique disruption with heterogeneous high signal on T2WI in 2 cases, multi-segment disruptions with heterogeneous high signal on T2WI in 2 cases. Type Ⅲ: With fracture in proximal tibia in 12 cases. MRI showed hyperintense signal only on T2WI in 1 case and multi-segment disruptions with heterogeneous high signal on T2WI in 11 cases. Type Ⅳ: With multi-fracture in both tibia and fibula in 7 cases. MRI showed multi-segment disruptions with hyperintense signal on T2WI in 7 cases. Conclusion MRI features of PMT injury mostly show multi-segment PMT disruptions with heterogeneous high signal on T2WI. |
