目的 采用血氧水平依赖功能MRI(BOLD-fMRI)技术评估丹参注射液对大鼠对比剂肾病(CIN)的保护作用。方法 将60只SD大鼠随机均分为丹参组和对照组进行BOLD-fMRI,分别于注射对比剂前、后连续注射丹参注射液及生理盐水7天(每天1次);于注射对比剂前、注射后20 min、24 h、48 h、72 h和7天点每组各取5只大鼠行BOLD-fMRI扫描,测量大鼠肾脏外髓部R2*值,后取大鼠肾脏行病理检查,评价外髓部肾小管损伤情况,分析和比较两组R2*值及病理损伤变化过程和差异,评价R2*值与病理损伤相关性。结果 两组大鼠外髓部R2*值于注射对比剂后20 min均增高,随后对照组先回落后复升高,丹参组持续回落;注射对比剂前及注射后20 min和48 h丹参组R2*值高于对照组(P均<0.05),注射后72 h和7天丹参组R2*值低于对照组(P均<0.05)。病理结果表明两组大鼠注射对比剂后20 min外髓部肾小管均出现损伤,注射后7天对照组病理损伤评分达最高,丹参组恢复至注射前水平;注射对比剂后20 min丹参组病理损伤评分明显高于对照组(P<0.01),24 h~7天丹参组病理损伤评分均低于对照组,除24 h外,余时间点两组间差异均有统计学意义(P均<0.05)。肾脏外髓部R2*值与肾小管损伤病理损伤评分呈正相关(右肾:r=0.58,左肾:r=0.52,左右肾合计:r=0.56;P均<0.001)。结论 丹参注射液对CIN具有预防和治疗作用,其效果可通过3.0T BOLD-fMRI进行检测。 |

Objective To explore the protective effect of salvia miltiorrhiza injection (SMI) for contrast-induced nephropathy (CIN) in rats using 3.0T blood oxygen level-dependent functional MRI (BOLD-fMRI). Methods Totally 60 rats were randomly divided into SMI group and control group (each n=30) and underwent BOLD-fMRI. The SMI and saline was administrated one time everyday before and 1—6 days after injection of contrast agent (CA) in the two groups, respectively. BOLD-fMRI was performed on 5 rats of each group before and 20 min, 24 h, 48 h, 72 h and 7 days after injection of contrast agent to measure the R2* value of kidney outer medulla and then the degree of renal tubule injury in the outer medulla was assessed by pathology. The change processes of R2* value and renal tubule injury were analyzed and compared between different groups and time points. The correlation of R2* value and renal tubule injury was analyzed. Results R2* value of both groups increased significantly 20 min after injection of CA, then falled-rised in control group and fall continiously in SMI group. R2* value in SMI group was higher before and 20 min, 48 h after injection of CA, and lower 72 h and 7 days after injection of CA compared with control group (all P<0.05). Pathology results showed that renal tubule injury occurred 20 min after injection of CA, which reached to the most serious degree in control group and recovered to the pre-injective level in SMI group 7 days after injection of CA. The pathological injury score of SMI group was significantly higher 20 min after injection of CA and lower 24 h to 7 days after injection of CA compared with control group (all P<0.05, except for 24 h). R2* had positive correlation with the pathological injury score of renal tubule (right renal: r=0.58, left renal: r=0.52, two renals: r=0.56, all P<0.001). Conclusion SMI plays an important role in prevention and protection of CIN, which can be detected by 3.0T BOLD-fMRI. |
