目的 采用二维超声心动图(2DE)获取健康孕妇左心房构型的相关定量数据, 并用体表面积对其进行标化, 进一步了解妊娠期妇女左心房构型发生的适应性改变。方法 采用2DE检测100名健康孕妇(早孕组、中孕组、晚孕组)及年龄相匹配的25名健康未孕妇女(对照组)的左心房构型指标。对比分析健康孕妇各组和对照组2DE测量的未经体表面积标化的左心房构型超声常规指标 以及经体表面积标化的左心房构型超声标化指标, 并观察左心房构型指标标化前后的变异系数。结果 随着孕周的增大, 健康孕妇左心房构型常规指标(LAD、LAV)及标化指标(LADi、LAVi)均呈现逐步升高的趋势, 在早孕、中孕、晚孕组间的差异均有统计学意义(P均 <0.05)。左心房构型常规指标的变异系数明显大于标化指标的变异系数。结论 健康孕妇心脏构型超声心动图标化指标稳定性优于常规指标, 可降低超声常规指标评价孕妇左心房构型变化的假阳性率, 其结果更具可靠性。 |

Objective To investigate the left atria configuration in the healthy pregnant women using two dimensional echocardiogram (2DE) according to body surface area, and to analyze stability of left atrial configuration between ultrasound conventional index and standardized index in order to explore adaptability change of the left atria configuration in the pregnant women. Methods The left atria configuration of 100 healthy pregnant women (the first trimester group, the second trimester group and the third trimester group) and 25 age matching healthy non-pregnant women were detected by 2DE. Left atria configuration parameters (including left atrial diameter, left atrial volume ) were analyzed. Standardized left atria configuration parameters (including left atrial diameter index, left atrial volume index ) by body surface area were studied, and the coefficient of variation of the left atria configuration parameters and standardized the left atria configuration parameters were observed. Results With the development of healthy pregnant women gestation, atria configuration parameters (LAD, LAV) and standardized parameters of body surface area (LADi, LAVi) increased gradually and there were significant statistical difference among the first trimester group, the second trimester group and the third trimester group (all P <0.05). The coefficient of variation of left atria configuration parameters was significantly greater than that of body surface area standardized left cardiac configuration parameters. Conclusion The stability of standardized parameters by body surface area in healthy pregnant women is better than the left atria configuration parameters, which suggests the standard treatment can reduce the false positive rate, and make result more reliable. |
