目的 评价能谱CT对胃神经鞘瘤(GS)和胃间质瘤(GST) 的鉴别诊断价值。方法 收集GS患者12例(GS组)和GST患者20例(GST组), 行平扫和三期增强能谱CT扫描, 分别在单能量图像和碘(水)基物质分离图像上定量分析CT值及碘(水)浓度, 定性分析其形态学表现。结果 与GST组相比, GS组CT表现多为圆形或类圆形软组织肿块, 倾向于胃腔外生长, 均匀强化, 坏死囊较少见, 最大径多 <5 cm, 较少形成溃疡(P均 <0.05)。GS组动脉期和门静脉期40-70 keV单能量CT值及碘浓度均低于GST组, 动脉期水浓度高于GST组(P均 <0.05);动脉期40 keV单能量图像上以102.58 HU为阈值和以碘浓度8.73(100 μg/cm3)为阈值, 诊断GS的敏感度、特异度均达100%。结论 能谱CT对GS和GST的鉴别诊断有一定价值。 |

Objective To explore the value of energy spectrum CT in differential diagnosis of gastric schwannoma (GS) and gastric stromal tumor (GST). Methods Twelve patients with GS (GS group) and twenty patients with GST (GST group) were enrolled. All patients underwent plain and three phases enhanced spectral CT scan. CT value and iodine (water) concentration on monochromatic and iodine (water)-based material decomposition images were quantitative analyzed respectively, and the morphological manifestations were qualitatively evaluated. Results Compared with GST group, GS group usually showed round or roundish soft tissue mass, exogastricus growth predilection, homogenous enhancement, rare necrosis and cystic changes, the largest diameter less than 5 cm and seldom ulceration on CT images (all P <0.05). The CT value on 40-70 keV monochromatic images and iodine concentration were lower in GS group than those in GST group in arterial and portal phase, but water concentration was higher than that in GST group in arterial phase (all P <0.05). The diagnostic sensitivity and specificity for GS were 100%, when the thresholds of 40 keV monochromatic images and iodine concentration was set as 102.58 HU and 8.73 (100 μg/cm3) respectively in arterial phase. Conclusion Energy spectrum CT have certain value for differential diagnosis of GS and GST. |
