      目的 分析腮腺Wathin’s瘤与多形性腺瘤声像图特征和临床特征。方法 选取于我院病理证实为腮腺Wathin’s瘤与多形性腺瘤的连续性病例47例,分别评价两种肿瘤的临床表现及病灶的声像图特征,Wathin’s瘤的独立危险因素分析采用Logistic回归分析。结果 两种肿瘤患者的年龄、性别构成、病灶大小、有无吸烟史、回声强度、回声是否均匀、是否存在无回声区差异有统计学意义(P均<0.05),其中患者年龄、肿瘤大小及吸烟史是腮腺Wathin’s瘤的独立危险因素,OR值分别为1.24、3.78和105.20。建立回归方程后,预测多形性腺瘤的准确率为87.5%,Wathin’s瘤的准确率为91.3%,整体模型的准确率为89.4%。结论 根据临床及超声特征可对两种常见腮腺肿物做出较为准确的定性诊断,可为外科制定手术方案提供更多帮助。
英文摘要:
      Objective To analyze the clinical and sonographic features of Wathin's tumor and pleomorphic adenoma of parotid gland. Methods Totally 47 consecutive patients with Wathin's tumor and pleomorphic adenoma of parotid gland proved by pathology were retrospectively recruited. The clinical data and the results of the sonographic examinations were reviewed and evaluated. The independent risk factors of Wathin's tumor were analyzed with Logistic regression. Results There were significant difference in the patients' age, sex ratio, size of the mass, smoking history, echo intensity, internal echoes distribution and the presence of anechoic areas between Wathin's tumor and pleomorphic adenoma (all P<0.05). The age of patients, size of the mass and smoking history were the independent risk factors of Wathin's tumor, and the OR were 1.24, 3.78 and 105.20. By using the prediction model, the accuracy of pleomorphic adenoma was 87.5% and 91.3% of Wathin's tumor. The overall accuracy was 89.4%. Conclusion Wathin's tumor and pleomorphic adenomaby can be made more specific diagnosis combined high frequency sonography and clinical features, which can provide more accurate information for surgical plan.
