| 目的 观察韦格肉芽肿（WG）胸部CT征象，并分析误诊原因。方法 回顾45例临床确诊为WG患者的临床及CT资料。结果 WG临床主要表现为多系统受累，上、下呼吸道受累最常见，胸部受累34例。胸部影像表现多样，多发空洞性结节、空洞性肿块最常见，空洞多不规则，洞壁厚薄不一，空洞可成偏心性、内缘模糊并坏死碎屑。结论 WG可累及多系统，以胸部损害最常见。多发大小不等不规则空洞性结节，洞内壁模糊或伴坏死碎屑是相对典型的影像学征象，确诊仍需组织活检。 |

| Objective To observe CT features of Wegener granulomatosis (WG), and to analyze the causes of misdiagnosis. Methods The clinical data and chest CT appearances of 45 patient swith WG were analyzed retrospectively. Results The patients frequently presented with multi-system involvement clinically, with the incidence of pulmonary involvement of 34 cases. The radiographic manifestations varied among the cases. Nodules and cavitations in the lungs were the most common signs.The cavitations were often irregular, difference of the thickness of wall, partial centrality, fuzzy of inner edge and necrotic debris. Conclusion WG can involve in multi-organ, and the lung is one of the most commonly involved organs. Multiple variable-sized cavitary nodules with irregular margins and contained necrotic debris are the relative typical findings. Biopsy is needed for definite diagnosis. |
