目的 探讨CEUS在肢体假性动脉瘤(PSA)诊断及评估中的应用。方法 收集27例肢体PSA患者,应用CEUS对PSA载瘤动脉瘤颈尺寸、瘤腔大小及附壁血栓情况进行评估,并与常规超声进行对比分析。结果 27例患者共检出33个PSA(残存假性瘤腔32个),其中1个瘤腔闭塞。CEUS假性瘤腔附壁血栓显示率为96.87%(31/32),常规超声附壁血栓显示率为75.00%(24/32),二者差异有统计学意义(P=0.026)。CEUS血栓表面显示率为90.62%(29/32),常规超声为46.87%(15/32),二者差异有统计学差异(P=0.0001)。CEUS示残存瘤腔最大切面面积为(11.99±6.49)cm2,常规超声为(8.32±7.96)cm2,差异有统计学意义(P=0.024)。CEUS载瘤动脉显示率为100%(27/27),常规超声为77.78%(21/27,P=0.023)。结论 CEUS在PSA的诊断及评估中具有其独特优势,应推荐广泛应用于临床。 |

Objective To explore the value of CEUS in diagnosing and evaluating limb psedoaneurysm. Methods Twenty-seven patients with pseudoaneurysm (PSA) were enrolled. The size of sac, neck and mural thrombosis of PSA was measured with CEUS, and compared with conventional ultrasound. Results Totally 33 PSAs with 32 false cavity were found in 27 patients. The display rate of PSA mural thrombosis was 96.87% (31/32) and 75.00% (24/32) diagnosed by CEUS and conventional ultrasound respectively (P=0.026). The display rate of PSA mural thrombosis surface was 90.62% (29/32) and 46.87% (15/32) by CEUS and conventional ultrasound (P=0.0001). CEUS showed the cross section area of residual cavity of PSA was (11.99±6.49)cm2 and conventional ultrasound was (8.32±7.96)cm2 (P=0.024). The display rate of parent artery with CEUS and conventional ultrasound was respectively 100% (27/27) and 77.78% (21/27, P=0.023). Conclusion CEUS has advantages in diagnosing and evaluating PSA and thus it should be recommended for clinical use. |
