目的 评价甲状腺超声影像和数据报告系统(TI-RADS)在甲状腺结节分级中的应用。方法 对1665个甲状腺结节进行TI-RADS分级,在超声引导下对结节行穿刺活检,根据病理结果,分析其诊断效能(敏感度、特异度、阳性预测值、阴性预测值、准确率)。结果 以TI-RADS 2、3级为良性病变,TI-RADS 4、5级为恶性病变,TI-RADS分级提示甲状腺结节良恶性的敏感度、特异度、阳性预测值、阴性预测值、准确率分别为95.35%(862/904)、16.16%(123/761)、57.47%(862/1500)、74.55%(123/165)、59.16%(985/1665)。结论 TI-RADS分级有助于规范甲状腺结节的超声描述及诊断,但仍需不断的补充、修改和完善。 |

Objective To evaluate the value of thyroid imaging reporting and data system (TI-RADS) in the stratification of thyroid nodule. Methods Totally 1665 thyroid nodules were evaluated by TI-RADS ultrasound stratification, and were compared with pathology results by biopsy. And the diagnostic efficiency, such as sensitivity, specificity, positive predictive value (PPV), negtive predictive value (NPV) and accuracy were analyzed. Results TI-RADS 2, 3 were defined as benign and TI-RADS 4, 5 were defined as malignant. The sensitivity, specificity, PPV, NPV and accuracy were 95.35% (862/904), 16.16% (123/761), 57.47% (862/1500), 74.55% (123/165), 59.16% (985/1665), respectively. Conclusion TI-RADS is helpful to classification descriptions and diagnostic ultrasound, but still constantly supplemented, modified and improved. |
