目的 通过MRI探讨淋巴瘤在眼眶部位的分布特征。方法 收集70例经病理证实的眼眶淋巴瘤(91只眼)患者的MRI资料,分析淋巴瘤在眼眶各区和不同结构中的分布特征。结果 25只眼(25/91,27.47%)病灶累及1个区、8只眼(8/91,8.79%)累及2个区、32只眼(32/91,35.16%)累及3个区、11只眼(11/91,12.09%)累及4个区,15只眼(15/91,16.48%)累及5个区;受累区域包括眶隔前区(52/91,57.14%)、肌锥外区(69/91,75.82%)、肌锥内区(54/91,59.34%)、眼球区(45/91,49.45%)和骨膜下区(14/91,15.38%)。淋巴瘤累及眼眶结构包括眼睑和结膜(52/91,57.14%)、泪腺(41/91,45.05%)、眼肌(75/91,82.42%)、视神经鞘(33/91,36.26%)和眼球内结构(1/91,1.10%);累及眼肌时以3条及3条以下为多(54/75,72.00%)。淋巴瘤累及眶周其他结构包括眶尖区(26/91,28.57%)、泪囊或鼻泪管(15/91,16.48%)、眶下裂(14/91,15.38%)、翼腭窝(10/91,10.99%)等。结论 淋巴瘤在眼眶内以累及多部位、多结构为特征,常累及肌锥内区、肌锥外区及眶隔前区。 |

Objective To explore the distribution of lymphoma in the orbit. Methods MRI data of 70 patients (91 eyes) with lymphoma in the orbit confirmed by pathology were retrospectively analyzed. The characteristics of lymphoma-involving distribution were assessed. Results One orbital space being involved was found in 25 eyes (25/91, 27.47%), 2 orbital spaces being involved were detected in 8 eyes (8/91, 8.79%), 3 in 32 eyes (32/91, 35.16%), 4 in 11 eyes (11/91, 12.09%) and 5 in 15 eyes (15/91, 16.48%). Lymphoma-involving orbital spaces included preseptal region (52/91, 57.14%), extraconal compartment (69/91, 75.82%), intraconal compartment (54/91, 59.34%), eyeball region (45/91,49.45%) and subperiosteal region (14/91,15.38%). Lymphoma-involving structure included eyelids and conjunctivas (52/91, 57.14%), lacrimal glands (41/91, 45.05%), ocular muscles (75/91, 82.42%), optic nerve sheaths (33/91, 36.26%) and intra-eyeball (1/91, 1.10%). In most of the eyes in which lymphoma involved ocular muscles, ≤3 ocular muscles (54/75, 72.00%) were involved. Lymphoma also involved other structures surrounding the orbit, such as orbital apex region (26/91, 28.57%), nasolacrimal duct (15/91, 16.48%), inferior orbital fissure (14/91, 15.38%), the fossa pterygopalatina (10/91, 10.99%) and so on. Conclusion Lymphoma in the orbit is characterized by more than one affected parts and structures. Moreover, the intraconal compartment, extraconal compartment and preseptal region are often involved. |
