      目的 采用MRI观察短暂极高海拔探险所致大脑皮质结构改变。方法 对28名健康青年在极高海拔登山前、后及1个月后随访进行常规T2W及全脑高分辨率三维T1W结构成像扫描,观察显著改变的脑区面积及其厚度,并应用Freesurfer软件进行统计学分析。结果 登山前、后及1个月T2WI均未发现异常;左、右侧大脑半球皮层总萎缩面积(平均萎缩率)分别为(3795.14±35.23)mm2 [(9.33±3.19)%]和(2175.42±26.15)mm2 [(6.49±2.76)%];左侧舌回皮层萎缩率与进入极高海拔持续时间呈弱相关,其余脑区则呈显著相关。结论 攀登极高海拔可引起大脑皮质萎缩,以优势半球明显,主要见于前额叶。

      Objective To observe structural alterations of cerebral cortex after single short-term extremely high altitude mountain-climbing with MRI. Methods Twenty-eight healthy mountain climbing amateurs who successfully completed mountain climbing underwent high resolution MR T1W and routine T2W scanning before, immediately and 1 month after mountain climbing. The data were analyzed using Freesurfer software. Cerebral cortical thickness and area were obtained and statistically analyzed. Results No changes were found on T2WI. The area of cortical atrophy (mean atrophy percentages) of left and right hemispheres was (3795.14±35.23)mm2 ([9.33±3.19]%) and (2175.42±26.15)mm2 ([6.49±2.76]%), respectively. Weak correlation with residence time was detected in left lingual gyrus cortex atrophy rate, while significant correlation was observed in other brain areas. Conclusion Cerebral cortex damages occur in extremely high-altitude mountain climbing, especially for cerebral cortex in dominant hemisphere, which mainly happened in prefrontal cortex.
