      目的 观察能谱曲线鉴别诊断肝脏小肿瘤的价值。方法 回顾性分析49例肝脏占位患者,其中肝小血管瘤(SHH)18例,小肝癌(SHCC)18例,肝小转移瘤(SLM)13例,均采用宝石CT能谱成像模式(GSI)扫描,获得门静脉期的能谱系列图像;观察SHH、SHCC、SLM的能谱曲线规律,分别选取50 keV和100 keV两点作为参考点,测量并计算3种病灶的能谱曲线斜率,并进行单因素方差分析。结果 SHH、SHCC、SLM的能谱曲线在40~80 keV能量区间内均呈明显下降趋势,80~140 keV能量区间内较平缓下降,且在40~80 keV能量区间内3种病灶的能谱曲线差异较大。3种病灶的能谱曲线斜率分别为2.77±1.03、1.99±0.42、0.80±0.34,差异具有统计学意义(P<0.05)。结论 能谱曲线及其斜率可以反映物质的同源性,对鉴别肝脏小肿瘤病变性质有一定价值。

      Objective To explore the value of spectral curve in differential diagnosis of small hepatic tumors. Methods A retrospective analysis of 49 patients with small liver occupying lesions was performed, including 18 patients with small hepatic hemangioma (SHH), 18 with small hepatocellular carcinoma (SHCC) and 13 with small hepatic metastases (SLM). Images of all patients were obtained with gemstone CT spectral imaging (GSI) scanning. According to the law of the spectrum curves about SHH, SHCC and SLM, 50 keV and 100 keV were selected as reference points, and then the rate of curves of these lesions were measured and calculated. One-way analysis of variance was applied to analyze the results. Results The energy spectrum curves of SHH, SHCC, SLM displayed trend of significant decline in keV energy interval of 40-80, while descending gently in keV energy interval of 80-140. The slope was 2.77±1.03, 1.99±0.42 and 0.80±0.34 respectively for three spectrum curves (P<0.05). Conclusion The homology of substance can be reflected by energy spectrum curve and its slope, thus contributing to differential diagnosis of small hepatic tumors.
