目的 观察不同WHO病理组织学分级胰腺神经内分泌肿瘤(pNEN)的CT表现。方法 回顾性分析经手术病理证实的23例pNEN的CT表现,并依2010年WHO病理组织学分级分为Ⅰ级、Ⅱ级、Ⅲ级进行对照研究。结果 23例pNEN中,WHOⅠ级12例,Ⅱ级8例,Ⅲ级3例。不同级别肿瘤性别构成差异具有统计学意义(P<0.05);但患者年龄构成、肿瘤大小、位置、形态、生长方式及有无坏死、囊变、钙化、内分泌功能差异均无统计学意义(P均>0.05)。增强后,不同级别肿瘤动脉期、平衡期绝对强化程度、病变与静脉期肝脏CT值差的差异有统计学意义(P均<0.05);但随级别增高,肿瘤强化程度未见规律性变化。结论 术前难以根据CT表现对pNEN进行分级。 |

Objective To observe CT features of pancreatic neuroendocrine tumors (pNENs) with different WHO histological grades. Methods CT features of 23 patients with pNENs confirmed by pathology were retrospectively analyzed. The patients were divided into 3 groups according to tumor's grade, i.e. gradeⅠ group, grade Ⅱ group and grade Ⅲ group. Results There were 12 patients in grade Ⅰgroup, 8 in grade Ⅱ group and 3 in grade Ⅲ group. No difference was found among patients' age, tumor location, size, tumor morphology, growth pattern, necrosis, calcification nor endocrine function (all P>0.05), but significant difference was detected between different genders (P<0.05). On enhanced CT scanning, statistical differences of absolute intensive degree were found between arterial phase and equilibrium phase, as well as CT value between different grades of tumor and the venous phase of liver, but there was no regular decline and rise with the increase of tumor's grading. Conclusion CT features can not help preoperative grading of pancreatic neuroendocrine tumors. |
