目的 探讨原发性肝血管肉瘤(PHA)的影像学特征。方法 回顾性分析6例经手术病理证实的PHA患者的MRI、CT、病理及临床资料。结果 6例PHA中,肿块型4例,肿块结节混合型1例,弥漫多结节型1例。CT:平扫均为低密度,4例密度不均匀,1例中心见片状略高密度;增强后1例动脉期呈边缘环状、中心网格状明显强化,周缘见晕环,静脉期延迟填充强化,2例动脉期呈多中心结节样明显强化,静脉期呈延迟填充强化,1例动脉期病灶中心明显点状强化,静脉期中心强化呈絮状及不规则斑片状,2例动脉期无明显强化,静脉期呈中心结节样或偏心性不规则斑片状轻度强化,延迟期呈不均匀低密度。MRI:平扫4例 T1WI呈低信号,T2WI呈不均匀高信号,DWI呈不均匀高信号;增强后1例动脉期呈边缘花环状、中心结节样明显强化,周围见高信号晕环,静脉期呈延迟填充强化,2例动脉期呈多中心结节样明显强化,静脉期呈延迟填充强化,1例动脉期呈明显中心点状强化,静脉期呈絮状及不规则斑片状强化。结论 PHA的影像学表现具有一定特征性,对术前诊断具有重要价值。 |

Objective To explore imaging features of primary hepatic angiosarcoma (PHA). Methods MRI and CT findings, pathological and clinical data of 6 patients with PHA proved by pathology were retrospectively analyzed. Results There were 4 massive type, 1 mixed type and 1 diffuse nodular type PHA. On CT, all 6 PHA were low-density, 4 were uneven density, while sheet slightly higher density could be found in 1 case. After administration of contrast agent, PHA in 1 case enhanced significantly with edge ring, the center got grid at arterial phase, with halo sign at periphery area, and delay filling enhancement occurred at venous phase. PHA in 2 cases significantly enhanced with multiple central nodular at arterial phase and delayed filling at venous phase. PHA enhanced significantly and punctately at the center at arterial phase, flocculently and irregular patchily at venous phase in 1 case, whereas did not enhance at arterial phase, but showed central nodular or irregular patchy eccentric mild enhancement at venous phase and heterogeneous density at delayed phase in 2 cases. On MRI, all 4 cases showed low signal on T1WI, heterogeneous high signal on T2WI and uneven high signal on DWI. After administration of contrast agent, PHA in 1 case enhanced significantly with edge ring, the center got grid at arterial phase with halo sign at periphery area, and delay filling enhancement occurred at venous phase. PHA in 2 cases significantly enhanced with multiple central nodular at arterial phase and delayed filling at venous phase. PHA enhanced significantly and punctately at the center at arterial phase, flocculently and irregular patchily at venous phase in 1 case. Conclusion Imaging features of primary hepatic angiosarcoma have certain characteristics, which are important to preoperative diagnosis of this disease. |
