目的 应用经食管实时三维超声(RT-3D-TEE)定量分析二尖瓣成形术对瓣环三维结构的影响。方法 将30例二尖瓣脱垂患者根据术前是否伴有左心室增大分为两组,A组为左心室内径正常患者,共10例,B组为左心室内径增大者,共20例。于二尖瓣成形手术前后分别行RT-3D-TEE检查,对比分析两组患者二尖瓣环结构参数变化。结果 ①与二尖瓣成形术前相比,术后A、B两组瓣环面积、周长、前后径均明显减小(P均<0.05),主动脉瓣二尖瓣环夹角与术前比较差异无统计学意义(P>0.05);②B组术后瓣环左右径、高度、高度/左右径比值明显减小(P均<0.05),而A组手术前后差异无统计学意义(P均>0.05);③术后B组瓣环面积、周长、左右径、前后径及高度变化率均明显大于A组(P均<0.05)。结论 二尖瓣成形术后,特别术前伴左心室增大时,瓣环部分三维结构可产生明显改变。 |

Objective To quantitatively evaluate impact of mitral valve repair and annuloplasty on mitral annulus geometry using real-time three-dimensional transesophageal echocardiography (RT-3D-TEE). Methods RT-3D-TEE was performed in 30 patients with mitral valve prolapse, including 10 with normal left ventricular end-diastolic dimension (LVDd, group A) and 20 with expanded LVDd (group B). The changes of mitral annular geometry after mitral valve plasty were analyzed. Results ①After mitral valve plasty, 2-dimensional area of mitral annulus (A2D), 3-dimensional circumference of mitral annulus (C3D) and anterior-to-posterior diameter of mitral annulus (DAP) significantly decreased in both group A and B (all P<0.05), while the aortic-mitral angles (θ) were not significantly different with those before (all P>0.05). ②The changing trends of anterolateral to posteromedial diameter of annulus (DALPm), annulus height (H) and H/DAlPm were significant different in group B after mitral valve plasty (all P<0.05), but not in group A (all P>0.05). ③The change rate of A2D, C3D, DAlPm, DAP and H in group B were larger than those in group A (all P<0.05). Conclusion After mitral valve plasty, some of mitral annulus structural parameters become obviously different, especially in patients with expanded LVDd. |
