      目的 应用1H-MRS,结合LCModel软件,对局灶性脑皮质发育不良(FCD)患者的脑代谢物浓度进行定量分析。方法 对具有癫痫病史、常规MRI诊断为FCD的患者行单体素点解析波谱序列扫描,测定脑内疑似FCD病灶及正常脑组织区域的脑代谢物。使用LCModel软件对13例扫描后接受手术治疗、且病理学确诊为FCD的患者的原始数据进行后处理,比较FCD病灶(病灶组)及其影像学显示正常的脑组织区域(对照组)的MRS数据。结果 病灶组N-乙酰天冬氨酸(NAA)及肌醇(Ins)浓度显著低于对照组(P均<0.01),甘油磷酸胆碱(GPC)浓度高于对照组(P<0.01),而两组肌酸(Cr)、磷酸肌酸(PCr)及谷氨酸+谷氨酰胺(Glu+Gln)差异均无统计学意义(P均>0.05)。结论 与正常脑组织相比,FCD病灶的NAA、Ins及GPC含量发生显著变化;1H-MRS 结合LCModel软件诊断FCD具有良好应用价值与前景。

      Objective To quantitatively analyze changes of metabolite contents in focal cortical dysplasia (FCD) with 1H-MRS and LCModel software. Methods 1H-MRS was performed on patients suffered from epilepsy and diagnosed as FCD by routine MRI. Single voxel PRESS sequence was used to assess the metabolic changes. Among these patients, 13 underwent surgical operation and were pathologically diagnosed as FCD, their data of spectroscopy were post-processed by LCModel software, and the differences of metabolite contents between FCD lesions (lesions group) and contralateral normal parts (control group) were analyzed. Results Compared with control group, the concentration of N-acetylaspartate (NAA) and myo-Inositol (Ins) were significantly lower, and of glycerophosphocholine (GPC) was significantly higher in lesions group (all P<0.05). However, the concentration of creatine (Cr), phosphocreatine (PCr) and glutamate+glutamine (Glu+Gln) were not significantly different between the two groups (all P>0.05). Conclusion Compared with the contralateral normal parts of brain, significant changes of concentration of NAA, Ins and GPC occur in FCD. 1H-MRS combined with LCModel software may have application value in diagnosis of FCD.
