| 目的 应用盆底三维超声断层成像(TUI)技术观察经阴道分娩后女性盆底耻骨直肠肌损伤情况.方法 选取经阴道分娩的产后女性80名(其中72名自然阴道分娩,8名接受产钳助产)行经会阴盆底三维超声检查,利用TUI观察耻骨直肠肌在耻骨支的附着情况,对撕脱进行诊断,并比较自然阴道分娩与产钳助产肛提肌损伤率的差异.结果 69名耻骨直肠肌完整.11例耻骨直肠肌出现撕脱,包括双侧撕脱4,右侧撕脱4例,左侧撕脱3例;其中7例自然阴道分娩,4例接受产钳助产,后者肌肉撕脱的发生率 明显高于前者.结论 应用TUI技术可以较好地观察产后女性盆底耻骨直肠肌撕脱情况;接受产钳助产者耻骨直肠肌撕脱的发生率高于自然阴道分娩者. |

| Objective To observe the puborectalis avulsion in females after vaginal delivery with tomographic ultrasound imaging (TUI) of pelvic three-dimensional ultrasound. Methods Totally 80 females after vaginal delivery were collected, including 72 spontaneous vaginal delivery and 8 forceps delivery. All female underwent transperineal three-dimensional ultrasound. TUI was used to diagnose puborectalis avulsion, and puborectalis avulsion rates were compared between spontaneous vaginal delivery and forceps delivery. Results The puborectalis was intact in 69 female. Puborectalis avulsion was found in 11 female, including 7 cases of spontaneous vaginal delivery and 4 of forceps delivery, and 4 bilateral avulsions, 4 right-sided avulsions and 3 left-sided avulsions were detected. The incidence of puborectalis avulsion in forceps delivery (50.00% ) was higher than that in spontaneous vaginal delivery (9.72%, χ2=6.746, P=0.009). Conclusion TUI can be well applied to observe the puborectalis avulsion in females after vaginal delivery. Puborectalis avulsion is more common in females after forceps delivery than spontaneous vaginal delivery. |
